GENERAL SUMMARY:

The Director Resident Services assists residents in identifying appropriate resources to maintain their highest level of independence and satisfaction to include problem solving for health, social, and emotional needs and coordinating support services.

Essential Job Duties:

· Develops, oversees, markets and coordinates community resident services programs. To include health fairs and health talks with the Wellness Nurse.

· Monitors and oversees any outside care providers working at the community.

· Assesses prospective residents for appropriateness of residency.

· Coordinates new resident orientation processes to ensure a smooth transition and appropriate socialization

· activities.

· Conducts family conferences to determine the amount of support services required to meet the resident's needs.

· Responds to emergencies, assesses situation, and alerts medical help when necessary.

· Addresses and follows up on all resident concerns, incidents, and accident reports in a timely manner.

· Contacts family members during times of emergency and other concern issues.

· Communicates on a regular basis with health care providers to monitor and assess health conditions, safety issues, and the needs of all residents.

· Communicates with other staff members concerning resident changes and building operation issues.

· Develops and maintains a positive image with local community. May attend social and civic affairs.

· Ensures residents are in the appropriate levels of care within the community and are receiving supportive services needed to obtain optimal levels of health.

· Collaborates with other members of the community team in identifying and recommending additional services or transitions within the continuum of care for residents with changing needs.

· Strives to achieve high levels of resident satisfaction.

· Coordinates and/or attends relative community meetings related to resident transitions/status updates, including the not limited to:

o Interdisciplinary Team Level of Care Meeting

o Daily Health center stand up meeting

o Care Coordination meeting

o Independent Living Resident Health Committee

o Director’s meetings

· Maintains awareness and promotes all internal services to promote resident retention

· Coordinates new resident orientation

· Interacts with the resident and family members when a change in the resident’s condition necessitates additional services or a physical move within the continuum of care

· Coordinates communication with physicians, families and appropriate staff regarding resident’s status

· Oversees admission/discharge to and from other healthcare provider agencies ensuring a plan of care is in place for all anticipated needs

· Coordinates services with other departments and/or external resources and involves other departments in programming as applicable

· Collaborates with health center staff to coordinate resident’s short-term and long-term

· Assists the residents with understanding of their specific health plan benefits

· Develops and maintains listing of internal and external personal service providers entering community. Evaluates these resources and ensures tracking of required documentation.

· Plans and coordinates health education and support groups for residents, family and/or staff.

QUALIFICATIONS:

· Graduate of accredited college or university.

· One to three years’ experience working in long-term care, post-acute care or other health care setting is preferred.

· Ability to problem-solve, make ethical recommendations, and define appropriate boundaries

· Excellent verbal and written communication skills

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Must be able to sit for up to 50% of the workday. Must be able to stand and walk through the community for up to 50% of the workday. Must be able to push or pull up to 50 pounds unassisted. Must be able to lift up to 30 pounds. Must be able to bend and reach. May be exposed to infectious and contagious diseases.

Salary: $60,000.00 - $65,000.00 per year
