Overview
Responsible for leading the practice of quality, including development and implementation of quality assurance and performance improvement plans, protocols, materials and procedures.
Responsibilities
Identifies performance metrics, monitors outcomes, and provides education and consultation for regional support staff and leadership in order to keep abreast of industry best practices.
Communicates effectively and frequently with clinical practice team on performance and progress toward company and regional quality goals, significant findings, problems, and changes in practice or policy.
Serves as a quality specialist resource for the company and specifically for the regional clinical support team.
Provides frequent and accurate accounting for areas of quality risk.
Supports regional recognition programs for quality outcomes.
Routinely submits statistical, outcome and trend reports to include, but not be limited to, Stars, value based purchasing, HHCAHPS, hospitalization rates, quality events and survey results.
Maintains quality systems and makes data driven decisions regarding improvement and focus for wound care.
Keeps abreast of all industry best practices and CMS guidelines.
Maintains a working knowledge of HCHB and company operations impacted by quality.
Performs other duties as assigned.
Qualifications
Required
Current, unencumbered state license to practice as a Registered Nurse or Physical Therapist OR Master's level degree or higher.
Five (5+) years' experience working in a clinical care setting with at least three of those years in a home health setting.
Two (2+) years' management experience in a home health company.
Excellent communication skills with the ability to drive change and influence without authority.
Multi-site and remote management experience.
Strong computer and software skills. Ability to utilize advanced technical equipment such as digital imagery programs.
OASIS certification - COS-C or HCS-O required within 3 months of hire. Preferred
Experience in large scale program development and staff education.
Ability to communicate effectively with agency staff, therapists, clinical managers, directors of operations, and other members of the health care team.
