Job Description
Kalix is seeking applicants to fill the Director of Quality Assurance position.


The Director of Quality Assurance (DQA) would be responsible for maintaining accreditation and licensure. You will coordinate the planning, implementation, and evaluation of quality assurance measures across all agency programs and components of service. You oversee all ongoing activities related to developing, implementing, maintaining, and complying with the agency's policies and procedures.


Essential Functions


Under the supervision of the CEO, specific duties and responsibilities include but are not limited to:


  • Coordination of behavioral support, to include development and monitoring of behavior plans, managing rights restrictions, and assisting with behavioral components integrated into support plans.
  • Assist with systems to ensure adequate documentation to support evidence of service delivery.
  • Evaluate and coordinate agency committees to ensure achievement of accreditation, certification, licensure, and standards of professional practice in compliance with current policies and procedures.
  • Other related duties as required or assigned.
  • Coordinate annual review and update of agency policies and procedures to ensure compliance with all standards for accreditation, certification, and licensure.
  • Submission of Plans of Correction and continued follow-up for programmatic site visits, including Title XIX.
  • Coordination of staff training programs required for program staff and agency-wide. Includes training programs for DSP/JCs, Service Assistants, Supervisors, and Coordinators. Includes required training and training identified based on agency need. Includes training provided by the state, selection of training materials from outside sources, attendance at training events, and training developed in-house. Includes compliance, record-keeping, delivery of training as appropriate, and development of training when needed.
  • Development and implementation of systems to review and respond appropriately to critical incidents including medication errors, reports of abuse/neglect/exploitation, and serious events.
  • Serve as the Privacy Officer, to include privacy risk assessments, integrating privacy into policies and forms, staff training related to privacy, monitoring compliance, and overseeing business associate agreements. Also includes documenting, tracking, investigating, and taking action on all reports of privacy violations.
  • Evaluate and coordinate agency efforts to ensure continued accreditation, to include completion of the self-survey, development of needed plans of correction, and coordination of identified staff training procedures.
  • Development and implementation of systems to assure that the individualized planning process is conducted in compliance with licensing standards and individual plans lead to improved outcomes in alignment with peoples’ needs and desires.
  • Coordination of agency record-keeping systems to ensure compliance with all required accreditation, licensure, and certification standards.
  • Development and implementation of systems to assure compliance with licensing requirements.
  • Development/coordination of ongoing quality assurance methods to evaluate all aspects of the habilitation service delivery system.


QUALIFICATIONS:


Minimum Qualifications: Graduate Degree from an accredited college/university in Social Work, Special Education, or related human service field. The position requires five (5) years of previous experience with developmentally disabled persons and agency administration in a similar setting with comparable responsibility.


REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:


  • Knowledge of industry accreditation and ICF/IID standards as well as state licensure requirements and agency policies and procedures.
  • Ability to prepare and report data, in oral and written form, with professional competency.
  • Knowledge of developmental disabilities.
  • Ability to plan, organize, implement and evaluate complex administrative responsibilities.
