Job Description

Scope of Position:

Provides overall Leadership, direction and strategic planning for Case Management and Social work services to promote and assure the advancement of coordinated care across the Care Continuum consistent with the vision, mission, and strategic goals of OSUWMC. This position will plan, develop, implement, and oversee programs and projects designed to evaluate and improve clinical quality, safety, resource utilization and operations of assigned departments. Provides oversight and direction to integrate services between all business units and service lines. Responsible for formulating and administering related organizational policies and procedures; manages relationships with key vendors; provides consultation and advice to internal business units, as well as to external stakeholders (employers and providers). In coordination with system leadership, assures that Care coordination services effectively and efficiently meet the needs of all internal and external customers and are compliant with regulatory groups. Assumes overall responsibility, authority, and accountability for the organization and operation of assigned departments in accordance with established organization standards.

Position Summary:

The Director’s primary responsibilities include management of all case management of patients from pre-admission through post-discharge associated with the OSU Wexner Medical System inpatient units, specialty outpatient and emergency departments. This position has responsibility for daily operations management and leadership in strategic planning/implementing case management and social service4. Management scope includes 4 acute care hospitals, 1 acute Rehabilitation Hospital and two emergency departments as well as relationships with key contracts and vendors. Management scope of acute care and inpatient includes care of patients at UH (Doan Hall, Rhodes Hall), Ross Heart Hospital, Brain and Spine Hospital, Dodd Rehabilitation Hospital and OSU East Hospital. This position has several direct reports including Clinical Case Managers and Social work, Business analytics and Team Leads as well as office staff and project management. In addition, the position works closely with physician and nursing leadership and physicians across the healthcare continuum. The position is accountable for budgeting and the financial management to the budget, patient satisfaction, human resources management, and process improvement. The position will ensure consistency in management of workflows, documentation, quality and outcomes for case management across the health system. Provides oversight to the development and implementation of innovative clinical outcomes research initiatives and facilitate individual and programmatic contributions to OSUWMC and individual Business Unit or Hospital Division Balanced Scorecards.

Minimum Qualifications

For Hire: BSN and/or Master of Social Work is required. Discipline-specific Master degree or MBA or MHA preferred. RN license required in State of Ohio or LISW-S in Ohio. Minimum of 10 years’ of progressive case management experience with 7 years of supervisory/management/leadership experience required.
