Job Description

Direct Support Supervisor

Pay Rate: $40,000 annually + $30/month cell phone stipend and mileage reimbursement

ESSENTIAL DUTIES:
  • Must have access to the internet to maintain communication and submit needed documentation electronically
  • Conduct classes, training and arrange opportunities for consumers according to ISP goals and objectives
  • Responsible to see that consumer files are maintained with current documentation, according to New Hope, FSSA, CARF and Medicaid standards
  • Prepare program in conjunction with Manager and assure that plans are followed
  • Assure Consumer dignity, confidentiality and safety within the program at all times
  • Maintain communication with Manager
  • Be willing to respond, when off duty, for emergency needs of clients and staff
  • Assure that all services meet quality standards as outlined by the Manager
  • Attend all meetings/staffing to include training, development, and in-services
  • Coordinate and attend meetings and conferences for consumers and New Hope
  • Ensure that all individual billing and related documentation is submitted to management per program expectations. End of business day for on-site services, 36 hours from shift completion for off-site services.
  • Assist in the establishment and implementation of individual goals and objectives
  • Responsible for assignment of staff on a daily basis to ensure maximum utilization of each consumer’s plan
  • Provide accurate and daily written documentation in a timely manner that addresses all program requirements
  • Provide services that are essential to reducing the effects of a handicap and/or disability experienced by the consumer as identified on the Individual Service Plan (ISP) and as directed by Manager or designee
  • Follows work schedule and completes assigned responsibilities
  • Assist in the responsibility for supervision of Direct Support Professionals in Habilitation entities including service delivery, hiring, training, performance monitoring and evaluation
QUALIFICATIONS:
  • Must possess a valid state driver's license, proof of insurance coverage, and have a vehicle that is reliable for transportation of consumers
  • Complete all annual tests and trainings
  • Possess or obtain appropriate licenses to operate facility vehicles for community outings
  • Experience working with people with disabilities
  • Excellent organizational skills
  • Respectful, friendly attitude toward people is required
EDUCATION/YEARS OF EXPERIENCE:
  • High School Diploma or GED required. Associates degree in a Social Service area preferred
  • Previous supervisory experience preferred
  • 1-2 years experience working with people with disabilities required
