Patient Care Coordinator – Rehab Medicine – Days

Across NewYork-Presbyterian/Weill Cornell Medical Center, Patient Care Coordinators are the guardians of continuity of care; they are the skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge while ensuring quality outcomes. As a Patient Care Coordinator at NewYork-Presbyterian, you'll serve as a highly valued and respected leader from day one, empowered to make amazing things happen for our patients and their families.

In this role, you will work on managing the insurance processes for patients on the Inpatient Rehabilitation Unit, including delivering regulated documentation, working with insurance companies on denials, extensions, peer-to-peer reviews, etc. In addition, you will coordinate the process and procedures of the Inpatient Rehabilitation Facility - Patient Assessment Instrument (IRF-PAI) data while also assisting the Operations Manager in providing regulatory updates and education for all staff on proper scoring of IRF-PAI and new IRF guidelines. You will also manage daily team rounds for patients and ensure all regulated documentation is in place. Leveraging your clinical experience, you will work with the physicians to ensure all relevant comorbid conditions are captured and help the Operations Manager to develop and monitor quality improvement projects for the Inpatient Rehabilitation Unit.

We’re seeking a change-agent—someone with the drive, tenacity and leadership necessary to advance the Rehab Medicine Department at one of the world’s most respected university medical centers.

This is a full time days shift position working Monday - Friday from 9am – 5pm at NewYork-Presbyterian / Weill Cornell Medical Center

Preferred Criteria

  • At least 2 years of experience with an inpatient rehab population
  • Previous experience with Medicaid and Medicare arrangements
  • Previous experience working in a hospital setting

Required Criteria

  • Understanding of Federal, State and Local regulations as they pertain to inpatient rehabilitation
  • Good communication, management and organizational skills and ability to work in a team setting
  • Must be proficient with Microsoft Word and Excel & Reporting/Analyzing Tools
  • Bachelor’s Degree in Rehabilitation Therapy specialty, or Bachelor’s Degree in Nursing, or a Masters’ Degree in Social Work from a recognized college or university
