The Coordinator of Quality Review coordinates the delivery of care and concurrent collection of outcome data for targeted patient populations. Works closely with physicians and clinical staff to support programs that encourage evidence-based standards of care for Lahey Clinic patients. The Quality Clinical Reviewer collects, aggregates, analyzes and reports data regarding the patient populations being followed to the appropriate clinical and administrative teams. Project manages initiatives for disease management.

:

Essential Duties & Responsibilities including but not limited to:

1) Participates in the development of methods for data collection and assessment for targeted patient populations.

2) Collects data on targeted patient populations through daily concurrent review as well as retrospective chart

review.

3) Interacts with caregivers to ensure that patients being followed meet goals and targets for timely delivery

of care according to current practice standards.

4) Enters data collected into appropriate databases.

5) Analyzes and reports data collected to appropriate committees and teams.

6) Acts as member or facilitator on hospital or medical committees, preparing agendas, minutes, etc.

7) Manages projects and processes for quality and patient safety.

8) Participates in policy development for committees and processes being managed.

9) Organizes and presents educational sessions related to projects and processes being managed.

10) Project manages disease management initiatives.

11) Performs other duties as assigned or directed to ensure the smooth operation of the department/unit

12) Organizational Requirements:

  • Performs all other duties as needed or directed to meet the needs of the department.
  • Participates in departmental and/or interdepartmental quality improvement activities.
  • Maintains strict adherence to the Lahey Clinic Confidentiality Policy.
  • Demonstrates an understanding of the job description, performance expectations, and competency assessment.
  • Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards.
  • Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals into daily activities.
  • Complies with all Lahey Clinic Policies.
  • Participates in and successfully completes Mandatory Education.
  • Maintains courteous and effective interactions with colleagues and patients.
  • Complies with behavioral expectations of the department and Lahey Clinic.

Minimum Qualifications:

  • Licensure, Certification, Registration: Current license to practice professional nursing from the Massachusetts Board of Registration
  • Experience: Minimum of three years of clinical experience, two years in case, quality or utilization management.
  • Education: Graduate of a state approved and/or accredited School of Nursing
  • Skills, Knowledge & Abilities: Excellent oral and written communication skills, demonstrated ability to commuicate with physician and senior leadership, excellent interpersonal skills, knowledge of quality improvement techniques, data collection and analysis.Project management skills required
