Job Description

Community Healthlink (CHL) recognizes the power of a diverse community and seeks applications from individuals with varied experiences, perspectives, and background. Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Community RN in Worcester, MA.


The Program for Assertive Community Treatment, or PACT, is an intensive and highly integrated evidence-based practice for community mental health services. Our team serves 80 individuals age 18 + who struggle with serious and persistent mental illness and whose symptoms result in serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.


Unlike other community-based programs, PACT is not a linkage case-management program that connects individual’s services. Rather, it provides highly individualized services directly to clients. PACT is multi-disciplinary team model who whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients. Services are provided in clients’ own homes and communities and are provided 24 hours a day, seven days a week, and 365 days a year. This program serves clients in Worcester and its surrounding areas.


The nurse on the PACT team provides nursing support to individuals served by the team to include but not exclusive to


  • Community outreach to clients with persistent and severe mental illness to provide mental and physical health support
  • Strong organization skills and ability to work with a team approach a must.
  • Medication management, health and psychiatric education, liaison with community providers
  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle
  • Various case management responsibilities including grocery shopping, transport to medical appointments, and securing housing and benefits
  • 75% of client contact happens in the community
  • Coordination with Primary Care, other health providers, and family when applicable
  • Work as part of a multidisciplinary team to assist clients to work towards wellness and independence in the community
  • Minimum of 3 years psychiatric nursing experience
  • Grade N06 Community RN Associates or bachelor's degree in nursing; Current MA RN license
  • Health and wellness activities such as community walks, providing dietary education, and encouraging personal hygiene
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
