About the Role

The Community Provider Partner will be responsible for managing community primary care provider relationships as well as outbound outreach to engage patients with TailorCare. They will be required to meet quality and production metrics in order to provide the highest level of customer service to every patient and community partner. Community Provider Partners are responsible for learning the outreach model and adapting it to constantly changing patient interactions. This position will be responsible for defining and executing a strategic plan to build high-quality, high-value relationships between TailorCare and the leading medical groups in the markets served. The ideal candidate must be goal oriented, innovative, and willing to adjust to a fast paced and evolving environment.


Primary Responsibilities

  • Adhere to HIPAA guidelines and regulations
  • Maintain and ensure the confidentiality of all Protected Health Information
  • Develop a thorough understanding of the TailorCare clinical program
  • Leverage reports and dashboards to prioritize telephonic and field-based outreach to patients
  • Accurately record patient information
  • Define and execute plan for operational and clinical integration with key primary care providers in the local market
  • Quickly establish rapport with patients and provider partners
  • Use critical thinking skills to create outreach strategies that are designed to maximize patient awareness and engagement
  • Effectively communicate the value of the TailorCare program to the target population and community physicians


Qualifications

  • Bachelor’s Degree (or higher) in Healthcare Administration, Business Administration, or social science related field
  • Experience communicating with a senior population over the phone, documenting patient interactions is a plus
  • Valid GA Driver's License and access to reliable transportation
  • 2+ years of experience in provider network, medical sales, provider practice sales, or key account management


Location

This role is WFH but must reside in the Atlanta area – this position will require visits to local community provider offices and thus also requires reliable transportation.
