Position Summary

Greene Science Center on Columbia’s Manhattanville campus, the state-of-the-art building offers an environment where the highest level of scholarly scientific research takes place on a daily basis, with the benefit of being located in the heart of New York, one of the world’s most vibrant and culturally rich cities.
Columbia University’s Wellness Center is home to Columbia’s Community Health Worker (CHW) Training Institute called Institute for Training Outreach and Community Health (InTOuCH). This program recruits trusted members from faith and community-based organizations to complete an eight-week, certified CHW training curriculum in stroke and cardiovascular disease prevention.
The Community Nurse Educator will have a strong understanding of the socio-cultural factors that contribute to health disparities, with a specific focus on African-American faith-based communities. Reporting to the Program Director, the Nurse Educator carries out the Wellness Center’s mission to expand health access to individuals by linking community members, faith and community based-organizations. This position is a non-clinical role and is part of a team that provides equitable health education and services to the community, and collaborates with internal and external partners.
Responsibilities
Responsibilities include, but are not limited to the following:
Implements evidence-based education programs, activities, and materials to promote stroke and cardiovascular disease prevention and other wellness center related programs.
Develops curricula that is reflective of evidence-based practice and uses the appropriate methods of teaching to CHWs and the wellness center staff.
Facilitates training to CHWs and the wellness center staff in comprehensive screening measures. e.g. point-of-care testing for cholesterol, blood glucose, and HbA1c; blood pressure measurement; BMI measurement -- height and weight measurement, waist circumference measurement; and health counseling.
Responsible for maintaining the limited service laboratory registration in accordance to the NYS Department of Health.
Performs comprehensive screening measures to walk-ins at the wellness center, including point-of-care testing for cholesterol, blood glucose, and HbA1c; blood pressure measurement; BMI measurement -- height and weight measurement, waist circumference measurement; and health counseling.
Assists in development of clinical protocols and procedures as requested following current health guidelines.
Coordinates and supervises CHW-led health screening and health fairs.
Attends to referral calls from screening events, connects clients to community resources and partner community-based organizations.
Maintains professional relationships with community partners to ensure the successful delivery of programming as well as in non-traditional work settings (public venues, informal gatherings, outdoor events etc.
Performs miscellaneous job-related duties as assigned by Program Director and PI.
Minimum Qualifications
3–5 years of related experience. New York State Registered Nurse (RN) license or Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) required.
Proficiency in Microsoft Office Suite (Word, PowerPoint, Outlook, Excel).
Must be available to work occasional early morning, evening or weekend days as required.
Must be within commutable distance for all in-person or hybrid roles.
Preferred Qualifications
Bachelor’s degree in Nursing and/or equivalent related experience is a plus.
Must have at least (1) year experience in home health, assisted living, hospital or long-term care environment and (2) years of community outreach experience.
Demonstrated knowledge of nursing practices, techniques and methods applied to health and wellness resident care; federal and state and local regulations.
Demonstrated knowledge of good assessment skills.
Demonstrates skill in navigating and communicating in diverse cultural settings.
Ability to handle multiple priorities.
Highly professional.
Possess written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
Competent in organizational, time management skills.
Demonstrates good judgment, problem solving and decision-making skills.
Eager to learn new applications is a plus.
