Position Summary


Your job in the department is to . . .


Be a front-line health worker who acts as a liaison, link, intermediary and navigator between physical and mental health and community resources to facilitate access and to improve the quality and cultural competence of service delivery.

Building individual and community capacity in increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, navigation, care coordination, social support, and advocacy. Works as part of an interdisciplinary team.


Essential Functions/Major Responsibilities


JOB FUNCTIONS


Direct Service


  • Screen individuals for social risk factors. Provides coaching and social support by utilizing motivational strategies to encourage individuals to obtain needed physical and mental health services. Links individuals to community resources as needed. Develops health and social management plans and goals with assigned individuals.
  • Provide direct service such as: basic screening tests, translation and interpretation, medical, mental health, 40% and social history, program eligibility and barriers to care.
  • Build individual and community capacity by increasing health knowledge and self-efficacy to individuals from diverse cultural, racial, and economic backgrounds through a range of activities such as outreach, community education, case navigation, care coordination, social support, and advocacy.
  • Participate in overall data collection and review of Social Determinants of Health programming. Works with team to coordinate and supply relevant information to clinic leadership and statewide partners.
  • Provide cultural mediation among individuals, communities and health and social service systems (e.g. educating individuals and communities about how to use health and social service systems, building health literacy, cross-cultural communication). Coaches patients in effective management of chronic health and/or mental health conditions and self-care. Assist patient in understanding care plans and instructions while motivating patients/clients to be active and engaged participants in their health and overall wellbeing.
  • Assist individuals with eligibility for Medicaid/Oregon Health Plan (OHP) and works to increase OHP members particularly in local marginalized and underserved community populations.


Teamwork


  • Establish positive, supportive relationships with team members. Openly, clearly, and respectfully shares and receives information, opinions, concerns and feedback in a supportive manner.
  • Provide support and advocacy during initial medical visits, or when necessary, to assure that the individual’s medical and mental health needs and required referrals are being conveyed and that health and social services plans are being followed up on, with both providers and clients.
  • Work collaboratively with co-workers, building bridges and creating rapport with team members across the county.


Documentation:
  • Use electronic system to enter, edit, and retrieve client information
  • Pull simple data reports from Electronic Health Records system.
  • Maintain accurate and up-to-date documentation in the electronic health record, including screens, services provided, release of information, and other documents acquired on behalf of the individual.
  • Contact agencies for records.


Other Functions


  • Attend team meetings, participates in quality improvement projects and attends trainings as needed.
  • Seek out opportunities and contacts to assist in building community relationships. Represents Lincoln Community Health Center at resource fairs and events.
  • Maintain productivity level standard as established for this position.

  • Performs basic arithmetic (i.e., addition, subtraction, multiplication, and division) and uses basic numerical concepts (e.g., whole numbers, percentages) to complete job tasks. Makes reasonable estimates of arithmetic results without a calculator.
  • Maintain client confidentiality in accordance with HIPPA and CFR-42.

Job Conditions and Physical Demands


WORKING CONDITIONS


Job Conditions: Works independently or with a registered Nurse in clinics, family’s home or other locations in the community.


Work Location: Lincoln County. This position will be based in Newport and/or Lincoln City; however, it requires the ability to work at any of our locations. This position also requires work in client’s homes and other locations throughout Lincoln County.


Travel: Local travel throughout Lincoln County required.


PHYSICAL DEMANDS


Physical capability to handle high mobility demands and complex personal interactions.


  • Physical Ability: Tasks involve the ability to exert very moderate physical effort in light work, typically involving some combination of stooping, kneeling, crouching and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (10-25 pounds). The ability to get into and up from a sitting position on the floor well as sit on a floor for an extended period of time. Walk on uneven ground and negotiate stairs carrying equipment.
  • Sensory Requirements: Some tasks require visual perception and discrimination. Some tasks require oral communications ability. Some tasks require the ability to perceive and discriminate sounds.
  • Environmental Factors: Tasks are regularly performed with exposure to adverse environmental conditions, such as dirt, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, machinery, vibrations, electric currents, traffic hazards, animals/wildlife, toxic/poisonous agents. Some tasks may be performed with exposure to violence, disease, or pathogenic substances. Tasks may be performed in environments that could have second-hand smoke or pets.

Lincoln County offers a comprehensive benefit package including vacation, holiday, and sick leave as well as medical, dental, vision which is paid at 90% by the County and 10% by the employee. The County's current contribution rate is 11%.


Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
