Job Description
Essential Duties & Responsibilities


  • Other duties as assigned.
  • Work with dental teams to complete outreach and follow-up for patients with referrals, care and outstanding treatment plans, and perform reminder calls as necessary to dental patients.
  • Link patients to appropriate community resources for health and health-related services consistent with regulatory, contractual and corporate requirements.
  • Serve as an internal patient referral contact for inquiries from members and communication with the care manager.
  • Provide basic assessment for dental needs in community settings; knowledge of dental diseases in children is strongly desired.
  • Coordinate health-related services to increase patients access to care and assist patient to adhere to scheduled appointments consistent with patients plan of care in conjunction with the care manager.
  • Coordinate with school officials to contact and schedule appointments for CPS students with urgent dental needs.
  • Cultivate relationships with referral sources and multi-disciplinary team that includes, but is not limited to: medical, dental, and prenatal providers, case managers, outreach staff, call center staff, and community partners.
  • Coordinate oral health education and outreach to schools and community organizations to increase referrals to an HHC dental practice.
  • Deliver oral health information using culturally appropriate terms and concepts.
  • Help individuals, families, groups, and communities access information regarding oral health and dental services.
  • Continually assess oral health needs for youth in identified communities; maintain records on outreach and educational activities.
  • Run clinical and data reports as needed to fulfill reporting obligations and maintain appropriate program surveillance.
  • Help people understand their oral health conditions and develop strategies to improve their oral health and to support healthier behaviors and lifestyle choices.
  • Participate in quality initiatives to foster standardization and integration of dental services and scheduling.
  • Provide informal oral health education/counseling, support, and follow-up.
  • Implement and oversee a referral process between dental and medical practices to support oral health care for specified patient populations
  • Inform/assist patients in understanding and utilizing the affordable care program (can further breakdown responsibilities - eligibility determination, tracking data).
  • Implement activities and provider training to improve identification of high-risk dental patients among medical patients.
  • Work with primary care providers, Dental Director, and Dental Clinic Administrator to help improve dental services for children.


Qualifications


  • Demonstrated cultural sensitivity to work in a low-income and racially and/or ethnically diverse environments preferred.
  • Ability to work a varied schedule with rotating hours and travel locally.
  • Knowledge and understanding of the benefits and restrictions of the specific care coordination activities.
  • Effective problem solving, organizational, and time management skills.
  • Highly energetic, motivated, resourceful, and self-directed.
  • Strong written and oral communications skills.
  • Outstanding customer service and interpersonal skills.
  • Ability to work independently and meet strict deadlines.


Experience/Training


  • High school diploma or equivalent.


Technical Knowledge


  • PC, email, facsimile machine, and computerized voicemail system.


Software Knowledge


  • Windows, MS Office (Word, Excel, Access, and PowerPoint)
