Agency Description

The Philly Counts office was originally created to support the 2020 Census. By working closely with community members and partners, this program increased understanding about the importance of the census, educated the public on how to participate, ensured responses from the public for a fair and accurate count, and supported filling census jobs.

The Philly Counts community engagement model has focused on leveraging community networks to further key administration priorities and to increase overall civic engagement in our communities to support a more equitable democracy in Philadelphia. This has included the trusted messenger model to assist the City’s COVID-19 response, mutual aid efforts, and the 2020 General Election.


Philly Counts is now helping direct community engagement efforts for the COVID-19 vaccine. In partnership with the Philadelphia Department of Public Health and other public and private partners, Philly Counts will leverage its existing network of staff and volunteers to provide COVID-19 outreach, education, organizing, and canvassing efforts to ensure that every neighborhood has 70% of their residents receive the COVID-19 vaccine.



This is a full-time, grant-funded position under the direction of the Philadelphia Department of Public Health and Philly Counts, and in partnership Health Federation of Philadelphia. The Community Health Worker will promote positive health outcomes through care referrals and information sharing, while also organizing their local community around the COVID-19 vaccine to improve access to the vaccine and increase vaccine uptake.

POSITION RESPONSIBILITIES:

  • Create and find ways to support the creation and ongoing facilitation of peer-to-peer support systems/networks
  • Provide one on one coaching and social support to CHW program participants
  • Provide referrals and improve support for COVID-related health concerns, including but not limited to:
    • Mental health resources for anxiety and depression
    • Referrals for support services for people affected by the increase in violence due to psycho-social stressors related to the COVID-19 pandemic
    • Referrals for support services for people affected by drug use due to the psycho-social stressors related to the COVID 19 pandemic
  • Support neighborhood canvassing efforts, community organizing and vaccine clinic coordination
  • Mental health resources for anxiety and depression
  • Engage directly with community residents and serve as a bridge between the Philadelphia Department of Public Health and the community, with a particular focus on communities of color.
  • Share resources on care for chronic conditions and improve access to city health centers and other federally qualified health centers.
  • Referrals for support services for people affected by the increase in violence due to psycho-social stressors related to the COVID-19 pandemic
  • Referrals for support services for people affected by drug use due to the psycho-social stressors related to the COVID 19 pandemic
  • Increase awareness and improve availability and access to COVID vaccines and boosters

Work-Life

Working hours are generally 37.5 hours per week. Our team is currently hybrid with both virtual and in-office days, however the position can opt to come in the office each day if desired


Qualifications


Strong candidates will come from the neighborhoods and communities they will serve and possess the following:

  • Knowledge of Philadelphia’s communities and being comfortable interacting with all different kinds of people with different backgrounds and opinions.

  • Must be willing to travel to residents’ homes to conduct home visits
  • Desire to use the knowledge, tools, and connections developed in this role to further your career opportunities in the future.
  • Strength in trouble-shooting and developing creative solutions.
  • High school diploma or GED required
  • Must have a valid driver’s license and access to reliable transportation
  • Desire to develop and practice community organizing skills

TRAINING & CERTIFICATION:

Those selected for this position will complete a 5-week Temple University’s Community Health Worker training program. Training program costs will be paid for by the City of Philadelphia. The Pennsylvania CHW Certification fees will also be paid for by the City of Philadelphia after the successful completion of the hour, supervision and training requirements. The Temple training program will provide critical knowledge and skills as they prepare to organize and engage with vulnerable communities around the COVID-19 virus, other chronic diseases, as well as engage with the trauma and mental health issues arising from the increase in violence and drug use stemming from the psycho-social stressors of COVID-19.

This position will report to the CHW Program Supervisor
