Job Summary
Reporting to the Senior Manager of Clinical Operations, the HealthyMatch Community Health Worker will primarily be working out in the community with specific target populations in the Pittsburgh area. CHWs will work closely with HealthyMatch RNs and LSWs to improve patient care and outcomes.
Duties/Responsibilities
  • Act as a client advocate and liaison between the caregiver, family, and community service agencies.
  • Attend regular staff meetings, trainings and other meetings, as requested.
  • Help clients set personal health related goals.
  • Follow up with clients via phone calls and home visits
  • Provide referrals for services to community agencies as appropriate.
  • Document accordingly in the EMR and other software no later than 24 hours after client contact.
  • Responsible for establishing rapport with clients and their caregivers
  • Work collaboratively with other Help at Home personnel assigned to the same client.
  • Manage assigned caseload of clients.
  • Help clients connect with transportation resources and provide appointment reminders when needed.
Preferred Skills And Abilities
  • Experience working with at-risk, elder populations
  • Excellent interpersonal, written, and verbal communication skills
  • Experience working as part of a broader health care team and leading by influence
  • Ability to work independently and as part of a broader care team as the situation requires
  • Organized, flexible and ability to multitask
Education And Experience
  • Case Management / Care Coordination background preferred
  • 1-2 years of experience directly related to the duties and responsibilities specified
  • Community Health Worker Certification preferred
  • Strong preference for experience with elder population in a home health care, hospice/palliative or facility setting
  • Bachelor’s Degree preferred
