Job Announcement
Position:

COMMUNITY HEALTH WORKER
Salary: $44,036 - $62,677

Community Health Workers (CHW) help individuals and their families navigate and access community services, other resources, and adopt healthy behaviors.
The CHW will help clients identify and address health goals. The CHW provides social support and advocates for clients’ health needs, and provides services
such as needs assessments, health education, outreach, support and coordination to obtain needed resources. Works under indirect supervision in community-
based settings, including clients' homes. Training will be provided upon employment. MDHA offers competitive salaries and an amazing retirement

REQUIRED EDUCATION, EXPERIENCE AND LICENSES:
• Resident of Nashville with good knowledge of the resources of the community
  • Experience in a community-based setting
Candidates with accreditation earned in foreign institutes are encouraged to apply.
ESSENTIAL FUNCTIONS
• Identify clients to participate in the program through direct outreach
  • Refer clients to clinics, community resources and other agencies to meet the client’s identified needs and follow-up referrals to completion.
  • Advocate for clients to ensure health care compliance.
  • Conduct client health needs assessments and maintain the assessment data.
  • Act as a health coach for clients to achieve desired health behaviors.
  • Assist clients in completing forms/applications, scheduling appointments required to receive needed services or community resource.
  • Assist clients in their homes and community. Communicate to clients the purposes of the program and the positive impact it may have on their well-being.
  • Provide support and coaching by motivating and encouraging people to obtain care and services.
  • Empower individuals regarding advocacy on behalf of themselves and their community through outreach and prevention.
  • Serve as a liaison and advocate between patients, providers, staff, and the community by facilitating communication and coordinating services.
  • Refers clients to transportation services and help address other barriers to services.
  • Document all client encounters; maintain comprehensive electronic client files, which include client notes, release of information, and assessments. Document
activities, service plans, and outcomes achieved by client in an effective manner.
  • Maintain HIPPA compliance at all times.
Other Functions
  • Answer phones and directs calls to appropriate staff and partners; answer general inquires
  • Work with other CHW team members to create a directory of community resources and share best practices
  • Plan, organize, and attend vaccination clinics and health fairs sponsored by MDHA
  • Participate in on-going trainings as needed.
  • Collaborate with supervisor and MDHA grant writer to support programming and provide insight to enhance potential grants

Knowledge, SKILLS AND ABILITIES
Behavioral Skills:
Agencies and resources related to health care delivery, health education, and health-related services in the Nashville area. Understanding of trauma-
informed care principles, navigate case management software packages, perform duties of the position with minimal direction and complete tasks in a time-
sensitive environment.

Technical Skills:
Strong interpersonal and communication skills and the ability to work effectively with a wide range of individuals from a diverse community. Advocating on behalf
of clients. Microsoft Office software including Excel, Word, PowerPoint, and Outlook.

Physical Skills:
Provide follow through to each client and their health care plan . Provide case management services to clients. Plan, implement, and evaluate individual client
care plans. Develop rapport and trust among clients. Communicate medical information and logistics to health care professionals and community partners.
Navigate case management software packages. Perform duties of the position with minimal direction and complete tasks in a time-sensitive environment.
