Job Description

Job Summary
Under the provisions of the grant, will serve as a vital link between CHEN’s congregants and the organization in support of outreach activities.

Responsibilities will include:
1) engaging with Health Liaisons in the congregations to participate in the surveys and assessments that will inform the organization of their specific needs
2) assist with facilitation of program delivery and communications with community stakeholders
3) disseminating education and awareness materials as part of overall marketing and to encourage congregant participation in health and wellness opportunities
4) coordinating logistics for activities taking place at CHEN congregations
5) working closely with the team members to develop innovative ways to address the social determinants of health within the assigned region. In support of accomplishing these activities, he/she will be available to attend weekly video and in-person meetings with the team to stay up to date on and discuss progress and interim findings.

Duties and Responsibilities

  • Complete administrative tasks on an ongoing basis to ensure proper and organized record keeping, working with staff members when/if necessary.
  • Deliver needs assessments to assigned member congregations within the network.
  • Facilitates communication and coordinate services between providers and the clients/patients.
  • Travels to congregations, community locations, various agencies, and other outreach destinations.
  • Participate in weekly outreach efforts to raise community awareness of available programs and initiatives.
  • Maintain consistent communication via a variety of mediums (phone, virtual platforms, in-person) to check up on their progress, answer questions, make personalized recommendations and changes to their programs if necessary.
  • Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, agency representatives, supervisors, and office staff. Works to reduce cultural and socio-economic barriers between clients and institutions.
  • Assists clients in accessing health related services at NGH and NHC, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
  • Continuously expands knowledge and understanding of community resources and services. Facilitates client access to and utilization of community resources, including locating housing, food, clothing, prenatal classes, parenting, financial services, transportation, workforce development, education attainment, and providers to teach life skills, and relevant mental health services. Assists with scheduling appointments and completion of applications for programs for which they may be eligible.
  • Organize community-based health events and deliver programming and initiatives aligned with the specific needs of the congregants.
  • Performs miscellaneous job-related duties as assigned.
  • Meet with clients, individuals, community organizations and congregations as needed to create health programs designed for individual use and for groups.
  • Data collection and management; documents all client encounters and contracts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.

Minimum Job Requirements

  • Associate degree with 3 years of experience; BA/BS in health/human/social services preferred.
  • Possession of a valid driver's license in good standing and reliable transportation
  • Must pass a pre-employment criminal background check and drug test.
  • Experience working with persons from diverse cultural and socio-economic backgrounds.

Knowledge, Skills and Abilities Required

  • Ability to collected, aggregate, and report data.
  • Creative and analytical thinking.
  • Ability to plan, implement, and evaluate programs, initiatives, and events.
  • Skill in use of personal computers, iPads/iPhones and related software applications, and the Microsoft Office Suite
  • Strong written and verbal communication skills. Ability to work effectively at all levels in a collaborative, customer service-oriented team environment.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services, Knowledge of community agencies and resources.
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.
  • Ability to prioritize and reprioritize quickly.
