Job Title: Community Health Evaluation Coordinator


Location: St. Paul, MN 55102-1062


Shift/Schedule: Clinics are held on weekdays, evenings, and weekends. Applicant must be available to work various hours and weekends/evenings.


Pay Rate: $20-$25/Hr.


Holiday Rate: $32-$37/Hr.


Hours Per Week: 40


Hours Per Day: 8


Days Per Week: 5

Duties


  • Clinics are held in community settings throughout the metro. Applicant must be able to travel to clinics.


The Community Health Evaluation Coordinator will play a key role in improving the health outcomes of the local community by supporting the implementation of targeted health programs to improve the health status of the community. The Community Health Evaluation Coordinator will lead the data collection, evaluation, and reporting for contracted vaccination initiatives, including COVID-19, Flu, MPOX, and other vaccinations. The role will require proficiency in data analysis and visualization tools to provide insights and recommendations to program leadership and superior communication skills. The role will also require frequent interaction with confidential and protected health data and registries such as the Minnesota Immunization Information Connection (MIIC).


The ideal candidate will also have the skills to support these programs' planning and implementation. In addition, research and analyze data regarding varying demographics, community health trends, and behaviors locally and nationally to support the community health needs assessments required by the Patient Protection and Affordable Care Act.


Skills: Job Expectations:


  • Provides technical resources and assists with program implementation at community events and clinics.
  • Monitors current trends and community needs and provides suggestions for change.
  • Evaluates clinical outcomes and data analysis for community clinical care programs to provide insights and recommendations for program leadership.
  • Assist in data entry-team scheduling and training.
  • Operates with a high degree of efficiency and accuracy a variety of equipment and software (PCs, Microsoft Suite, Excel, Outlook, etc.)
  • Creates data tracking tools such as dashboards, surveys, and databases to facilitate program evaluation and reporting.
  • Provides support to other selected community health programs as needed.
  • Willingness to do administrative tasks such as data entry and record-keeping, as well as the ability to learn new skills such as statistical analysis and health informatics tools.
  • Assists in developing reports and presentations for program leadership, community partners, and grant funders to communicate program outcomes and progress.
