Job Description
Job Summary Provides personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort


Job Duties


  • Skin care/nail care
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as
  • Attends in-service programs to meet compliance requirements. Identifies learning needs to Agency
  • Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per Agency requirements—at least
  • Maintains confidentiality in all aspects of the
  • Assistance in ambulation or exercises
  • Bathing
  • Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and Agency policy).
  • Performs simple procedures as an extension of therapy services under the direction and supervision of the
  • Performs personal care activities, including but not limited to
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational
  • Responsible for completing all mandatory and regulatory training programs
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case
  • Oral hygiene
  • Meal preparation/feeding
  • Range of motion exercises
  • Dressing
  • Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse or Therapist. Follows reporting guidelines in the Care
  • Shaving
  • Laundry
  • Perform other duties as assigned
  • Light housekeeping
  • Shampooing
  • Performs household services essential to health care at home, including but not limited to


Attendance Requirements


  • Must be able to work nights and weekends as required
  • Must be available, and on-time for scheduled work shifts


Educational Requirements


  • Successful completion of a formal certification training program and/or a written skills test and competency evaluation


Experience


  • Minimum of six (6) months of work experience in a supervised setting, preferably health care


Credential/Licensure Required


  • Licensed driver with automobile insured in accordance with state and/or Agency requirement sand in good repair. Good driving


Special Conditions of Employment


  • Initial and continuous exclusion and sanction/disciplinary monitoring

Job Summary Responsible to maintain accurate and complete patient and employee records. To perform data entry, billing and payroll processes according to company policy and procedures.


Job Duties


  • Applies the policies and procedures of the agency and the rules and regulations of state and federal regulatory agencies and other certified agencies in providing
  • Responsible for completing all mandatory and regulatory training programs
  • Scan and uploads admission consent and other paperwork into Agency software
  • Maintain a professional and quality image at all
  • Reports complaints made by patients or families to the appropriate persons
  • Distributes routings, correspondences,
  • Accepts other assignments as deemed
  • Participates in on-call rotation as appropriate.
  • Files and maintains agency policy manuals and all other master manuals.
  • Identifies and responds appropriately to emergency
  • Generate billing log.
  • Gather forms for
  • File accurately using chart order or file
  • Set up files for new patients and employees and enter into
  • Participates in orientation of new staff if
  • Perform other duties as assigned
  • Ensure growth and profitability of the company through the responsible use of company
  • Willingly accepts direction from the Administrator, Director of Nursing, and Case
  • Adheres to established personnel
  • Ensures that care is provided with respect for patient
  • Maintain ongoing communication with co-workers and participates in staff meetings.
  • Answer the phone within three (3) rings and take messages
  • Reports all accidents and incidents
  • Track and maintain medical and office supplies and reorder as
  • Follows standard precautions and infection control
  • Working knowledge of computer
  • Produce census list, and other lists as required.
  • Adheres to established personnel
  • Meets or exceeds established productivity
  • Ensures protected health information is kept confidential
  • Responsible for scheduling of business unit personnel, as

Educational Requirements


  • High School Diploma


Experience Preferred


  • Prefer experience in an office setting


Physical Requirements


  • Primarily medium in nature with exerting up to 35 pounds of force to lift, carry, push, pull, bend or otherwise move objects, involves sitting, walking, standing, driving, talking, hearing, reaching. Ability to work at a desk for up to eight hours per day. Involves prolonged phone use.
