Job Description

Organization:

PAMF-Palo Alto Medical Foundation SCD

Position Overview:

Works independently and in consultation with physicians and other health care providers across the health care continuum to ensure appropriate medication therapy for individual patients. Uses patient interviews and counseling, reviews the medical record and medication profile, consults with physicians, teaching, staff education, medication use evaluation, adverse reaction monitoring and resolves medication-related problems. Interventions target high risk population through the provision of medication reconciliation and comprehensive medication use management in focused disease states. Assist with care coordination, disease state education and monitoring to achieve optimal outcomes and is focused on chronic disease state management as part of a coordinated team.

EDUCATION

Equivalent experience will be accepted in lieu of the required degree or diploma.

Doctorate: Graduate of an accredited school of pharmacy.

CERTIFICATION & LICENSURE

PHARMD-Licensed Pharmacist

OR PHARMR-Registered Pharmacist

SKILLS AND KNOWLEDGE

Comprehensive knowledge of current pharmacological/biopharmaceutical principles, pharmacotherapy, medical terminology, pathologies, disease states, and related information for a medical management plan.

Knowledge of acute and chronic disease and its effects on patients and caregivers.

Able to use professional judgment and clinical expertise in the daily solving of complex problems and in the provision of clinical information within the scope of practice.

Ability to adapt to changes in the work environment or work processes in a timely, positive and effective manner.

Demonstrated commitment to team work by willingly accepting responsibilities and performing assignments that support the team.

Demonstrates knowledge of total quality management through its precepts, skillful workplace applications and continuous organizational improvement.

Exhibits a customer/supplier philosophy that emphasizes both internal and external relationships; identifies both customer and supplier needs/expectations and strives to exceed them.

Ability to participate in, and encourages others to utilize creative and innovative approaches to accomplish tasks.

Demonstrated responsibility for ongoing personal development, professional growth and continuing education.

Interpersonal skills collaborating with physicians, patients, families, care givers, service providers, and health care team.

Written and verbal communication skills.

Requires a basic working knowledge of legal requirements and accreditation standards including National Association of Boards of Pharmacy (NABP), The Joint Commission (TJC), Title XXII, United States Department of Homeland Security (DHS), Drug Enforcement Administration (DEA), Food and Drug Administration (FDA) and United States Pharmacopeia (USP).
