The Care Center is a team of Care Coordinators, and Clinical Outreach Specialists (LPNs & RNs). The team is often the first and/or last touch with the member/patient; establishing rapport and leaving a positive participant experience. The Clinical Outreach Specialist engages with participants before and after they have completed a Health Assessment by a Matrix Network healthcare professional while in their home, on a Mobile Health Clinic, via telehealth visit, or in the employee health and wellness setting.


Using a member-centered approach, the Clinical Outreach Specialist Specialists are also responsible for answering calls from participants who have been or are preparing to be tested or vaccinated for COVID-19 through Matrix Medical Network. This nurse will ask appropriate questions related to scheduling or promoting vaccinations and or scheduling monoclonal antibody (MAB) infusions or injections and providing education if needed on expected frequently asked questions or potential common side effects. Telephonic support will be used to gather pertinent information, address questions/concerns, and provide educational and/or medical guidance regarding the vaccination and or MAB program. Post-assessment, the Clinical Outreach Specialist is also responsible for following up with members to provide lab results, address questions/concerns, and provide guidance on follow up critical for the next step in the member’s care.


RESPONSIBILITIES


  • Provide members with useful information and answer questions associated with the COVID-19 vaccination and or its affiliated side effects
  • Provide members with useful information and answer questions to help prepare for upcoming clinic or home visits
  • Demonstrates excellent customer service with his/her actions being consistent with Matrix’ Mission, Vision, and Core Values
  • Periodically called to duty in the field to complete Quality Visits within local market
  • Complete telephonic health assessments for health plan members across the country
  • Provides local community resource pool to refer members who need ongoing assistance to achieve health goals
  • Documents all member contact, follow up actions and resolution of issues into the current clinical documentation system
  • Some weekend coverage required
  • Have the ability and knowledge to recommend an appropriate level of care, based on these details, and possibly point patients toward additional resources
  • Builds and nurtures professional and effective relationships with all members of the member’s health care team, both internal and external
  • Works within established criteria for response time and deadlines related to communication with health plans, members, and primary care providers.
  • Engages members into the Matrix programs by conducting telephonic outreach and identifying health benefits to participation
  • Abides by all Matrix Medical Network privacy and confidentiality policies when accessing or communicating member information
  • Participates in all mandatory compliance and other training programs, seeks guidance for compliance-related concerns and adheres to all applicable laws, regulations and Matrix Medical Network policies and procedures
  • Reviews and communicates medical records and pertinent test results to members, PCPs, field providers, and specialists within the member’s care team
  • Other duties as assigned
  • Provides comprehensive condition-specific self-management training, education and coaching
  • Documents all member/provider contact, follow up actions and resolution of issues into clinical documentation system
  • Ensures all necessary lab results are reported properly and accurately to the primary care provider and/or the participant in a timely manner according to established policies
  • Triage alerts of Care Management patients based off of real time biometric data and daily care pathways
  • Acts as liaison between the Care Center, field clinicians, primary care providers and health plans
  • Assess participants and recommend the proper level of care by asking participants about their symptoms, existing health problems, and any relevant history of medical issues
  • Contact members with positive test results regarding contact tracing


Must Haves


  • Ability to build rapport and maintain positive, pleasant demeanor with all levels of internal and external contacts
  • Exceptional telephonic communication skills with health plan members or caregivers
  • Maintain professionalism with members, caregivers, and other health care team members at all times
  • Minimum two (2) years of nursing experience
  • Being comfortable with phone conversation, email, and fax as the main means of communication
  • Proficiency in Microsoft Office (Word, Outlook, Excel) and the internet
  • Ability to effectively work within a group dynamic
  • Ability to provide telephonic support and education to members in a way they are able to understand
  • Must have experience working with the Microsoft Office Suite (Outlook, Word, Excel)
  • Proven time management and organizational skills
  • Ability to work independently, coordinating, and assimilating multiple pieces of data to accomplish required tasks
  • Effective written communication skills
  • Ability to assess when escalation of items is necessary and escalate to the appropriate resource
  • Professional telephone and email etiquette
  • Maintains nursing license in good standing
  • Possess unquestionable personal and professional ethics, in concert with company standards
  • Current unrestricted licensure to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state(s) to which the candidate is assigned
  • Telephone triage experience preferred
  • Ability to prioritize activities in a rapidly changing environment
  • Consistent and effective follow-up skills
  • Ability to work in diverse, multi-cultural environment and be respectful to health plan members’ cultural and socioeconomic characteristics
