Overview
Clinical Case Manager (RN)
(Nursing Supervisor)
This leadership opportunity involves managing a caseload of medically fragile patients while ensuring the clinical care delivered by our field staff meets or exceeds the Aveanna Clinical Standard of Excellence. Our ideal candidate will be outcome driven and able to thrive in an always-changing environment. The administrative component of this job will consist of internal case management, potentially assisting with hospital discharges, our start of care process and nurse competency validation. The field-based aspect of this position will create opportunities for you to spend quality time with field staff in patient homes developing, mentoring and providing professional guidance.

Schedule:  Monday – Friday *paid, rotating on-call (may also include evening/weekend visits)  
Work Location:  Combination of Fort Lauderdale Office and Corresponding Patient Homes  
Compensation: $60,000 base salary plus monthly bonus potential, cell phone and mileage reimbursement

Responsibilities Of The Nursing Supervisor
  • Participating in the release of our patients from the facility to their home through “Start of Care Assessments” and “Re-evaluations” 
  • Ensure adherence to State, Federal, Local and Aveanna policies and procedures 
  • Collaborating with the operations and business development team 
  • Promote and enhance the level of clinical expertise of staff to provide safe and quality nursing care for our patients 
  • Review physicians’ orders and update the plans of care and documentation, as necessary 
  • Assist with Interviewing, selecting, training and validation of new clinical staff 
  • Internal case management, evaluation and development of nursing talent 
Qualifications Of The Nursing Supervisor
  • Must have reliable transportation, valid driver’s license, and pass MVR check 
  • Must have at least 1 year of hands-on nursing experience 
  • Must be able to travel to patients’ homes in designated territory 
  • Must have good standing license (RN) in the state in which the clinician will practice 
  • Must be willing to supervise nurses providing in home skilled nursing care to infants, children, adolescents and/or adults 
  • Current CPR certification from AHA or ARC

Responsibilities & Qualifications
Position Overview
The Nursing Supervisor provides overall management and supervision of client care activities within the home care setting. Work in conjunction with other team members to ensure appropriate level of staffing and coordination of care in order to meet client goals. This position requires an ability to be flexible and work with all members of the health care team and provide ongoing supervision, clinical education, support, and evaluation to the nursing staff.
*At Unskilled Locations - This position will only supervise unskilled roles, such as HHAs, CNAs, and Direct Care Workers.*
Responsibilities
  • Participate in call for after hour’s client care
  • Provide nursing updates and obtain re-authorization for continued care
  • Promote and manage expectations and satisfaction with internal and external customers
  • Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered in a manner sensitive to socio-cultural diversity
  • Ability to assess clients and provide direct client care as needed
  • Assume responsibility in coordinating care to assigned clients, establishing a goal directed care plan from admission to discharge which includes a comprehensive ongoing assessment of clients’ needs
  • Provide ongoing supervision, orientation, training, education, and evaluation of clinical field staff
  • Promote an environment of quality and safe client care through participation, development, and adherence to the QA plan and associated activities and metrics
  • Participate in employment decisions affecting nursing staff, including hiring and termination as appropriate
  • Identify professional practice standards within the organization and identify areas of strengths as well as areas for professional practice development
  • Ensure availability and proper operation of necessary equipment and supplies related to patient care
  • Contribute to nursing education and professional development of staff, students, and colleagues
  • Perform on site supervisory visits to assess client, family, environment, and clinical care givers and complete follow-up documentation
  • Evaluate the quality and effectiveness of nurse practice and nursing services, analyzing appropriate data and information to identify opportunities for collaboration with all stakeholders in order to improve services and patient outcomes
  • Provide direct client care as needed
  • Provide best practice in delivery of nursing care to the appropriate population and adhere to the standards of professional nursing practice
  • Maintain a professional demeanor consistent with registered nurse standards of practice
  • Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice
Requirements
  • RN licensure in designated states as appropriate
  • Valid Driver’s license and Acceptable MVR
  • Diploma, Associate, or Bachelor degree in nursing from state accredited RN program
  • Clinical or other work experience as per state and/or federal laws and regulations
Preferences
  • If supervising Private Duty has a minimum of two years’ experience in private duty, home care, or health care and the knowledge, experience and ability to effectively administer the private duty program preferred
