The CHHA- Nurse Care Manager role at the Hebrew Home at Riverdale by RiverSpring Living’s Certified Home Health Care Agency, a world-renowned long-term care organization located in the - North Bronx, is responsible for home visits, providing assessment for patients in their homes. Ideal candidate will be eligible for a $15,000 (less taxes) sign on bonus.

The Nurse Care Manager is responsible for home visits providing assessment and direct nursing care of patients in their own homes as well as the direction of paraprofessional staff and coordination of services provided by the interdisciplinary team of physical therapists, occupational therapists, and social workers.

The ideal RN candidate must be a dedicated and compassionate clinician and possess a BSN, current NYS RN license and registration, and have home care experience with the working knowledge of Home Care Home Base EMR is preferred. Strong organizational, communication, writing, and customer service skills are essential. Must be competent with Microsoft Office, and have reliable transportation to conduct home visits.

Compensation: $105,000 min. to $110,000 max. Annually
