Job Description
Brighton Hospice is a rapidly growing industry leader. We are looking to add positive, compassionate, and knowledgeable individuals to our growing team. The goal of hospice care is to improve the quality of a patients last months by offering comfort, dignity, and care wherever the patient resides.
*This position will be covering patients in Macomb County*
  • This role will be Monday-Friday 8am-5pm*
On an average day as a Chaplain (referred to internally as a Spiritual Care Clinician) you will:
  • Conduct a personal ministry to patients/families when other resources are not available or appropriate, including conducting funerals and memorial services when requested
  • You will have the opportunity to collaborate with our clinical teams (physicians, social workers, nurses, therapists) to contribute to that mission. These passionate team members will depend on you to a ssess and support the spiritual and bereavement needs of patients and their families . Offer short-term individual counseling, crisis intervention
  • And perform additional, related duties as needed
  • Develop an individual plan for spiritual care in conjunction with the hospice interdisciplinary team, and when necessary in coordination with the patients/families/caregiver, church, clergy or spiritual family in order to accomplish a holistic plan of care for each patient/family/caregiver.
  • Document all contact with patient/families, including spiritual assessment, home visits and important telephone calls in relation to the Plan of Care in the patients clinical record.
  • Be available to other care team members for bereavement support

Desired Skills/Qualifications

  • Clinical Pastoral Education Training and/or Spiritual Care Counseling Training preferred
  • Are able to demonstrate the level of knowledge and skills normally associated with a bachelors degree, including knowledge of Hospice, death, dying, grief and bereavement
  • Have a valid drivers license, auto insurance, and reliable transportation for the purpose of traveling to our patients
  • At least one year of hospice experience
  • Passion for providing the best possible care to patients and their families
  • Comfortable in an interfaith setting
