We are seeking a motivated and independent individual to join our team as a Case Monitor. In this role, you will be responsible for overseeing cases and ensuring the well-being of our clients. Your primary focus will be on monitoring treatments and responses, advocating for necessary services, and assessing patients through intakes. Additionally, you will be responsible for maintaining accurate records and performing various administrative duties. This position also requires occasional on-site visits to clients and patient care.

Job Responsibilities:

• Maintain clients' records by reviewing case notes, logging events, and tracking progress
• Occasionally visit clients on-site to provide additional support
• Monitor cases by observing and evaluating treatments and responses
• Advocate for needed services to ensure the well-being of clients
• Perform general administrative duties to support the team
• Assess patients through intakes to gather necessary information

Please note that this job requires patient care experience and the ability to travel up to 60 miles from downtown Williamsport. Additionally, there may be occasional on-call responsibilities performing patient care in the event of a call off. If you do not meet these requirements or are unable to fulfill the travel and on-call aspects of the role, we will be unable to offer you the position, however we do have other positions available that may be a better fit and we encourage you to apply.
