Job Description

ACRS seeks a full-time Case Manager I who will responsible for planning, organizing, and facilitating youth groups and training, recruiting and engaging youth in the implementation process of the Pathways to Our Future program, providing outreach and resource education to the target population, and coordinating the day-to-day operation of the program. The role involves providing quality case management/individual support services to the target population and collaborating with other professional agencies. Attention to cultural relevance, competency, diversity, equity, and inclusion is essential in providing quality services to the target population.

ESSENTIAL JOB FUNCTIONS:

  • Prepares concise, complete, and timely records according to agency policy, meets contract and agency requirements, and ensures quality care and services to the target population. Completes other reports and letters pertinent to the program.
  • Other job-related duties as assigned by management.
  • Participates in supervision and training as required by the program.
  • Provides quality case management/individual support services to the target population. Consults, coordinates, and collaborates with other professional agencies.
  • Plans, organizes, and facilitates youth groups and training; recruits and engages youth in the implementation process of the program. Provides outreach and resource education to the target population. Provides quality services with attention to cultural relevance, competency, diversity, equity, and inclusion; coordinates day-to-day operation of the program and collaborates with schools and other youth serving providers in development of the program. Prepares and completes program reports in a timely manner.
