UW MEDICAL CENTER - NORTHWEST SOCIAL WORK has an outstanding opportunity for a PER DIEM CASE MANAGEMENT ASSISTANT (Day Shifts). The Case Management Assistant will be responsible for working closely with the Social Worker, Nurse Care Coordinator, Physician, and the patient to arrange follow-up appointments post-discharge and assist with coordination of the discharge planning process for patients.

RESPONSIBILITIES:

Arrange hospital discharge follow up appointments and referrals

Assist in arranging home services, such as home health, wound vac, or in-home caregivers, based on information obtained from clinical staff

Assist in arranging practical needs and emergency assistance, such as transportation, parking, or local lodging. Demonstrate problem solving skills in working with resources outside the medical center

Process and send clinical updates to payors

Research and update community resource information as needed

Participate in the training of Social Work Students and new employees

Document interventions in electronic medical record

DESIRED:
Ideal candidates have experience working in a medical setting, demonstrate strong multi-tasking and organizational skills, oral and written communication, and computer proficiency. Must work well with a variety of persons and teams and possess excellent customer service skills.

REQUIREMENTS:

Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent OR an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
