The Caregiver or Care Partner provides direct personal care and supervision to the residents in the specialized memory care unit in the community. The Care Partner promotes resident wellbeing and satisfaction through support with activities of daily living and communicates with other departments to ensure resident needs are met.
Responsibilities of the Care Partner:
Assist with activities of daily living, including following community protocol, licensing regulation and guidelines for both resident and employee safety.
Find opportunities to give the resident choices throughout the day and allow them to be as independent as possible with their personal ADLs.
Ensure that the policies and procedures are effective and enforced to support the health and safety of the residents.
Follow safety guidelines in the community, including proper lifting techniques and universal precautions when providing care to the residents.
Follow the schedule of duties as provided on the individual care plan for each resident.
Function as a team, assisting coworkers as the need arises.
Monitor resident activity, food intake, functional status, psychosocial status, taking action as required to promote resident well-being.
Document resident status change and communicate changes to Memory Care Director.
Assist with resident activities/programming as requested or assigned.
Set-up tables in preparation for meals, serve meals, and clean-up following meals including clearing dishes, wiping tables and chairs, and sweeping.
Serve the residents 3 snacks and beverages each day including those who are in their rooms during snack time.
Assist with laundry by immediately rinsing and washing soiled items as needed.
Housekeeping duties for common areas including, but not limited to, dusting, straightening up, wiping down tables and chairs, cleaning bathrooms, replacing paper towels and toilet tissue, cleaning activity areas, sweeping, mopping, and completing assigned cleaning schedules.
Collect garbage from assigned resident rooms daily. Immediately remove incontinent supplies that are wet or soiled.
Assist in updating the assessments and care plans as needed.
Review and utilize the Community’s disaster preparedness plan for staff and residents to follow in case of fire, explosion, natural disaster, or other emergency, including interruption of electrical power supply, gas-heating supply and water supply.
Act immediately on any resident crisis, following protocol and basic first aid training.
In the event all assigned duties cannot be completed timely, ask for assistance and report to the Memory Care Director.
Refrain from gossip, but promote open communication between health care professionals, families, residents, and staff.
Attend monthly staff meetings and complete all necessary in-service training.
Read all shift change acknowledgement notes.
Review and follow the employee handbook guidelines, including dress code, conduct, scheduling, etc.
Properly complete and chart all ADLs in a timely manner,
Other duties as assigned by the Administrator. Position Requirements:
Must be at least 18 years of age.
Must have an acceptable criminal background record check.
Must keep a current certification in emergency first aid and CPR.
Must have a clear tuberculosis screening upon hire to assure that the employee is free of diseases communicable within the scope of employment.
Must have a physical examination prior to employment and as required by the Community after an injury or illness to assure that the employee is physically qualified to work.
Must be able to bend/stoop, climb, reach above shoulder level, kneel, balance, push, pull, squat, crouch, lift, carry, and move up to one hundred (100) pounds.
Must have training in emergency evacuation procedures, medical and social needs of the residents, resident’s rights, abuse and neglect reporting, infection control, medication assistance, dementia, and courses relevant to the job duties.
Must obtain at least twenty-four (24) hours of continuing education the first year and sixteen (16) hours of continuing education courses each year thereafter.
Must take all dementia related training courses and additional training specific to the memory care residents.
Must be able to read and comprehend simple instructions, short correspondence, and memos. Able to write simple correspondence. Able to effectively present information in one-on-one and small group situations to customers, clients, and other employees.
Must attend mandatory staff meetings.
Employee must not be under the influence of alcohol or other substances while engaged in any work-related activity on behalf of the Community.
Employee must wear a name tag at all times while on the property or engaged in any work-related activity on behalf of the Community.
Able to work various schedules and shifts as needed.
