Job Description
The Care Manager Specialist will coordinate the design, planning, evaluation, and integration of clinical education, orientation, and professional development activities so as to maintain consistent standards of practice and competencies in the delivery of Care Management. In addition, this role will work with Decision Support to transform the data into actionable information in order to influence clinical practice. Excellent presentation and communication skills are required.


  • Facilitates the orientation, staff development, continuing education, competency validation, patient


education for Care Management staff.


  • Facilitates opportunities for interaction between external partners and care managers
  • Transition/discharge planning
  • Serves as a liaison by establishing and maintaining open lines of communication with other hospital departments, the consumer, and the community.
  • Assumes patient load as needed
  • Facilitates CEU offerings for care managers and social workers with external partners.
  • Utilization review and management
  • Applies effective teaching-learning methodologies to care management staff development for all new and existing care managers and social workers
  • Facilitates the development of strong leadership behavior in the department
  • Promotes and contributes positively to intradepartmental and interdepartmental relationships.
  • Assists with unit recruitment and retention initiatives.
  • Follow yearly license renewal for compliance
  • Incorporate Midas and Meditech modules and functionality in the orientation materials and annual updates in staff education.
  • Performs monthly or quarterly compliance audits as directed
  • Defines and coordinates orientation/onboarding needs for all new department employees, as well as accepted students. For students, individualized orientation, with a preceptor, evaluates the orientee’s progress and provides ongoing constructive feedback to both orientee, their preceptor, and the outcomes manager.
  • Assist the department leadership in the assessment of new departmental employee learning needs and development of employee orientation and education plans to ensure consistency in implementation validation and documentation methodologies.
  • Adheres to Hospital Dress Codes, Safety Policies, and Procedures.
  • Psycho-social support to patients and families
  • Consistently provides opportunities for interaction with employees to discuss professional growth, personal concerns, and educational goals.
  • Displays a caring and courteous attitude and represents the hospital in a positive manner to all persons noted above.
  • Maintains the confidentiality of patient and department information without infractions.
  • Demonstrates expert team-building skills, problem-solving abilities, and conflict-resolution strategies as they provide leadership not limited to their nursing area.
  • Collaborates with department leadership and support staff using indicators, such as regulatory agencies, new policy development, new technology, patient case study, and quality assurance monitors, to prioritize educational needs.
  • Monitors care management staff workflow and documentation in support of the following’
  • Promotes direct caregivers as valuable assets to the hospital.
  • Seeks opportunities to enhance ongoing education needs within Care Management.
  • Compliance based requirements (IM Notice; MOON; Choice; Discharge Planning)
  • Care coordination
  • Develops and implement department-specific staff educational activities for Care Management.
  • Demonstrates regard for the dignity, respect, and safety of all patients, significant others, visitors and hospital personnel as defined in the philosophy of the organization.
  • Participates in meeting the Goals and Objectives of DCH, Care Management to help reduce length of stay, and prevent readmissions.
  • Maintains all current nursing home, home health, durable medical equipment, and hospice lists for ready access by the care managers and social workers for posting on the Intranet CM site.
  • Assesses and identifies staff ability for critical thinking and multitasking.
  • Assesses staff educational needs, develops a plan, implements and evaluates the effectiveness of the plan, in conjunction with the performance management process.
  • Collaborates with departmental executive leadership in developing department standards policies, procedures, protocols, documentation, forms, and transitions.
  • Serves as a role model and promotes the staff’s effective communication and conflict resolution skills, reflecting quality service attitudes and keeping consumer satisfaction in mind.
  • Assumes patient load at times of high census and staffing challenges
  • Provides input and assists director/managers in the annual review of department Standard of Practices, Scope of Service, and performance improvement initiatives in conjunction with other departmental work teams.
  • Contributes positively to the teamwork of the department by assisting other staff, contributing ideas, and helping to resolve problems.
  • Provides constructive and timely feedback on employee performance to employee and management.
  • Demonstrates clinical expertise by serving as a resource and role model for department personnel.

Registered Nurse or Licensed Social Worker with the following


  • Must be able to read, write legibly, speak, and comprehend English
  • Bachelor of Science in Nursing or Social Work preferred
  • Strategic planning experience preferred
  • Master’s degree in nursing, social work, business, or hospital administration preferred
  • 5 years RN or SW experience in a hospital setting preferred
  • Certification in Case Management preferred (ACM, CCM, CMAC)
  • Current RN or SW Alabama licensure
  • Public speaking and/or instructor experience preferred
  • Five years’ case management experience required
