  • Opportunity to work in a dynamic, fast-paced and innovative care management company that is transforming the delivery of kidney care.
  • Full Time Permanent Position for a Bilingual (English-Spanish) Nurse Practitioner
  • Competitive Salary + Bonus and Excellent Benefits
  • Monday – Friday, 8AM-5PM (Exceptions when patients request a visit after 5PM)
  • Work with underserved populations.
  • Max of 6 Patients a Day for Telehealth
  • Max of 4 Patients a Day for Home Visits
  • Focus on Home-Based Dialysis Patient
  • Full Time Permanent Position
  • 3-4 Days of Home Visits (within 1-hr radius of assigned territory) and 1-2 Days of Telehealth


Responsibilities


  • Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first.
  • Maintain quality of care and productivity standards as defined by the practice.
  • Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning. 6. Complete all documentation and paperwork in a timely manner.
  • Conduct assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur in-home.)
  • Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.
  • Document items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan.
  • Prescribe medications, order tests, and collaborate with patient’s physician.


Requirements


  • Must have access to transportation, a valid drivers license, and car insurance
  • Experience conducting annual wellness visits, comprehensive visits and appropriately charting HCCs
  • Proof of complete COVID vaccination
  • Experience practicing in managed care, value-based context required
  • Experience in end-of-life discussions, palliative care and hospice preferred.
  • Ability to do daily travel (within a 1-hr travel radius from main location) to patients’ homes
  • Sound understanding of all federal and state regulations including HIPAA and OSHA
  • Ability to work without direct in-person supervision and practice autonomously
  • 2+ years of experience as an NP
  • Current and unrestricted DEA Certification
  • Must be proficient with medical instruments and equipment required by the work
  • General knowledge of risk adjustment
  • Active and unrestricted New Jersey Nurse Practitioner license
  • Board Certified by ANCC or AANP
  • Experience in kidney disease and control of related co-morbidities (HTN, Diabetes, CHF) preferred
  • Bilingual in English and Spanish
