Children and Family Health Institute at Visiting Nurse Association Health Group supports people of all ages with more than 20 programs and services. We strive for every child and family in New Jersey to be happy and healthy by linking vital community resources to those who need them. Each year, more than 54,000 parents and children rely on the Children and Family Health Institute to access the care and services they need to achieve their best level of health and well-being.


Responsibilities


Community Health Worker InCK will help families in Monmouth/Ocean County identify strengths and challenges and carry out the Goals of the Integrated Care Plan(ICP). Drawing o n a strong network of NJ InCK partners and using new technology for communicating and making referrals, they will ensure the children/youth are connected to appropriate Core Child Services including physical and behavioral health care, child care and preschool, school delivered services, maternal child health programs, food and housing supports, legal services, and the child welfare system as needed. Community Health Workers are intended to be familiar with the geographic area to which they are assigned and to have cultural affiliations and in some cases language capacities that allow them to best serve the families and youth with whom they work.


Qualifications


The ideal candidate for the Spanish language Community Health Worker will have the following qualifications:


  • Excellent verbal and interpersonal communication skills
  • Education: High school diploma or equivalent; some college a plus
  • Ability to use new computer systems and mobile devices, including using Apps, databases & Microsoft Office
  • Car, driver’s license, and insurance required.
  • English language writing skills sufficient to clearly and accurately document interactions with families and youth in the NJ InCK Care Coordination and Information and Referral Systems.
  • Strong organizational and time management skills
  • Experience: 2-3 years of regular work experience, ideally involving services to children and /or youth

Working Conditions/Physical Demand: Working indoors at VNA locations and with travel to community locations to meet families/youth and providers of Core Child Services. Occasional participation in community events that may be held out of doors.
