Primary Role


Aim to promote behavior change in an effort to prevent new HIV infections, often by reducing sexual risk. Focus on management of patients with mental illness and addictions. Candidate is responsible for assessing clients for trauma who are at increased risk of HIV and develop care plans. Reports to Integrated HIV Supervisor.

Essential Job Duties


  • Advocate for patient needs and services and provide assistance to members in navigating the healthcare system
  • Engage members routinely telephonically and in-person via home/facility visits to coordinate services, community resources, and provide health education
  • Patient care coordinators are liaisons between patients and the healthcare system
  • Ensure that patients receive the care they need and that they understand their medical condition, medications, and other instructions
  • Ensure completion of required touch points and care coordination activities and document all activities within the member clinical record and/or population-based software
  • Initiate care planning and implementation of a comprehensive care plan and addresses medical
  • Assist patient with follow up for preventive health and screening tests
  • Create, review and revise Care Plans with a focus on health education and improving health outcomes
  • Complete trauma and behavioral assessments for the purpose of identifying patient care needs
  • Focus on reducing hospital admissions through coordinated care and effective discharge planning with other team members
  • Identify early risk factors and targeted condition management interventions as necessary
  • Provide education and assist with follow up appointments how to access appropriate care
  • Collaborate with the member's PCP and other care team members to deliver and coordinate quality care and services

Requirements


Qualifications:


  • Minimum of two years of employment experience relative to Substance Abuse/Mental Health Prevention or Substance Abuse/Mental Health Treatment Services is required.
  • Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
  • Must have a current (within 12 months) Tuberculosis (TB) skin test report upon employment and provide an update annually.
  • Bachelor’s Degree in social work or related health discipline and two years of directly related experience in HIV/Hep C is required.
  • Must have a valid and unrestricted New Jersey driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid NJ driver license during employment.
  • Must obtain a CPR Card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.
