Position Focus:
Administrative Responsibilities: Provide collaborative clinical leadership for an internal medicine primary care practice with a focus on excellence in quality of care, process efficiency, team development, and patient experience. Estimated at 30%-time commitment. Provide working leadership for a multi-disciplinary internal medicine team in accordance with the principles of a patient-centered medical home. Create an atmosphere of mutual respect, responsibility, accountability, growth, and cooperation between all team members (MA, RN, APRN, PA, MD, receptionist). With other department leaders, establish clinical and operational quality standards and goals in accordance with organization-wide principles and standards. Cultivates a high-quality team of clinical providers and directs the activities and professional development of team members including measures to promote retention and mitigate burnout. Work with clinical team to enhance accountability for team patients and ensure high level performance on established metrics. Responsible, with nursing leadership, for day-to-day team function and workflow to include access to appointments, cross coverage, performance management, oversight, feedback, and annual performance evaluation for team clinicians (MDs, PAs and APRNs.). Convene and lead regular team meetings in collaboration with nursing leadership; Participate in regular department leadership meetings. Participates as a member of Chiefs group, lead or serve as member on committees and projects related to quality and safety, research and education, patient experience, team-based care, panel management, and operational efficiency at the departmental or organizational level. Provide coverage for IM Chief as needed.
Clinical Responsibilities: Provide seamless, coordinated, and compassionate primary care to a panel of adult patients as part of a multidisciplinary team in the Department of Internal Medicine. Estimated at 70%-time commitment. Provide medically appropriate care that is patient centered, evidence-based, compassionate and cost-effective Serve as a standard of clinical competence, life-long learning and responsibility. Meet departmental standards for quality of clinical care, productivity and service. Define and document patients’ clinical problems and reach appropriate diagnoses; plan and execute therapy in a timely manner; keep comprehensive records of care; communicate to patients the results of testing or procedures in a timely manner. Provide same-day and urgent care as needed under departmental service and scheduling standards; participate in cross coverage of absent colleagues according to departmental standards; participate in weekend on-call rotations. Provide consultation and teaching as needed for midlevel clinicians; coordinate the internal medicine residency training and medical student primary care clerkship experience in the department with the administrative team. Interact with nurses, medical assistants, and other front-line staff in a professional, cooperative and supportive manner. Demonstrate commitment to the welfare of Yale Health patients and the department. Provide care for a panel of patients and consultation or backup for team and departmental colleagues; demonstrate flexibility to meet changing clinical demands. Regularly attend and participate in department meetings and committee assignments; Meet regularly with Department Chief and Manager.
