Salary: Competitive

Position Summary: Provides case management services to program participants to assure that their needs are being met. Develop a trustful and confidential relationship with clients wherein related service issues and needs may be comfortably resolved.
Position Responsibilities
  • Assist the families in completing personal service workers/employer applications.
  • Must carry a cell phone to be available to families.
  • Participate in improvement activities using total quality management structure.
  • Complete the Risk Assessment Tool annually.
  • Maintain current with all program and agency policies and procedures.
  • Other duties as assigned by Supervisor.
  • When applicable, access vendors and complete paperwork for the one time funding for the customer.
  • Complete all Home Based documentation including progress notes for each contact, and following established guidelines.
  • Complete billing sheets in a monthly basis, with a minimum of three hours per month within time frames.
  • Build appropriate relationships with all families and referral sources.
  • Implement and enforce agency policies and procedures on client service delivery. Work effectively to resolve client disputes or complaints relevant to agency service providers or policies.
  • Make referrals and linkages to requested services. Evaluate customers’ service needs.
  • Respond to phone calls in a timely manner.
  • Update program documentation annually, including releases of information and service authorizations.
  • Schedule bi-monthly visits with clients. Complete their annual Individual Service Plan (ISP) review; establish goals, and update for changes in conjunction with Community Alternatives Unlimited (CAU) and other Individual Service and Support Advocacy (ISSA) providers.
  • Adhere and enforce agency safety procedures. Assist in the evacuation of all program participants during a fire drill, tornado drill or other civil emergency drills according to the department evacuation plan.
  • Complete the Inventory for Client and Agency Planning (ICAP) annually.
  • Participate in agency staff meetings as well as other meetings required by funding agencies.
  • Conduct regular case review to assure that files are kept up to date in accordance with agency and funding source requirements.
  • Complete time sheets and submit to accounting as requested; resolve pay issues with appropriate agency.
  • Participate in fulfilling funding source requirements.
  • Maintain an active caseload of a maximum of 40 cases.
Requirements
Essential Skills and Experience:
  • Excellent verbal and written communication skills in Spanish and English.
  • Management skills: the ability to organize and manage multiple priorities .
  • Computer proficient .
  • An ability to work collaboratively with other health and human service providers to affect a continuum of care for agency clients.
  • Must have valid Illinois driver’s license and ability to meet agency insurance requirements.
  • An ability to provide services to the monolingual Spanish client.
  • High performance teams and a strong team player .
  • Commitment to company values .
  • Minimum of one year experience working with persons with Developmental Disabilities.
  • Planning skills: an ability to think ahead and plan over a given time span .
  • Must be able to work evening hours, and travel within the city and surrounding suburbs.
  • Must have a car.
  • Bachelor’s degree in social work or related field is required.

  • Previous experience in case management services.
