Job Description
Brief Description


Purpose of Job


This position is responsible for (clerical) functions related to processing of health information records. These functions may include collection and reconciliation of discharges; prepping, scanning, and indexing encounters into the medical record system; analysis of records for completion; assisting authorized callers with information access.


Job Functions


  • Indexes records into the electronic system.
  • Performs any document deletions or additions to electronic records when needed.
  • Reconciles the records received against the listing of activity for that patient type on a given date and ensures that all records are received.
  • May be required to follow up on physician delinquencies.
  • Assists callers in accessing medical information following established procedures and HIPAA requirements.
  • Preps records for scanning.
  • Scans records into the electronic system following established procedures.
  • Performs other duties as assigned.
  • Assists medical practitioners in the use of the electronic system to access information and complete deficiencies.
  • Analyzes records for deficiencies and creates deficiencies in the electronic systems following established procedures.
  • Collects records of recently discharged/treated patients from various locations within the organization.


Minimum Education/Work Experience


  • High school graduate or equivalent


Knowledge/Skills/Abilities


  • Knowledge of basic computer programs; spreadsheet, word-processing
  • Ability to meet and maintain the necessary background checks as aligned with position functions
  • Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above
  • Keyboarding/typing skills

  • Attention to detail
