Job Description
he Admissions Nurse is responsible for admitting all new patients/residents to the facility while ensuring the nursing operational standards of the facility are met in accordance with current federal, state and local standards governing the facility, and as may be directed by the Director of Nursing Services or her designee, to ensure that the highest degree of quality care is maintained at all times.


LTC/STR experience

Qualifications:


  • Must have knowledge of care planning, EMR & MDS.
  • Positive attitude toward the elderly and sick.
  • Must show ability to read and write in a legible and understandable manner.
  • Must be able to demonstrate leadership and supervisory skills.
  • Prior experience a plus
  • Must be knowledgeable of nursing and medical practices. These include procedures, laws, regulations and guidelines governing long-term care.
  • Must speak and understand English.
  • Must be at least 18 years of age.
  • Must be a Registered Nurse in good standing and meet all applicable federal and state licenser requirements.


Essential Job Functions


Administrative Function: Duties


  • Perform head to toe assessment of new residents.
  • Complete all paperwork for the Nursing Admission Assessment.
  • Ensure all paperwork for the admission is complete and signed off on.
  • Completes the Pressure Ulcer Risk Assessment / Braden Scale. If high risk ensures all preventative measures are implemented.
  • Administer 1st dose of PPD within 24 hrs and records on Immunization Record and Med sheet.
  • Must have strong communication skills.
  • Enter all data into Electronic Medical Record.
  • Obtain orders from MD and ensures accuracy of Admission Orders in EMR.
  • Comply with all state requirements and facility policies, procedures and standards of conduct.
  • Establish and maintain communication processes with all patient care.
  • Completes the Admission Nursing Note.
  • Ensure Initial/Interim Care plans are put in place. Fall Risk, Skin Alt-Potential, Pain-specific resident conditions (pressure ulcer, post-op, infection, etc).
  • Reports to the RN Supervisor on duty.
  • Obtain consents: Pneumovax / Flu and places in the patients’ chart.
  • Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
  • Complete the Fall Risk Assessment and intervention care plans as indicated (sensors, alarms, etc).
  • Must meet all new residents/patients upon admission.
  • Communicate and collaborate with the interdisciplinary team and or physicians.


Physical and Sensory Requirements:


  • Ability to communicate with residents, families, personnel and support agencies.


2 Ability to remain calm under stress.


3 Must be able to bend, lift and push at least 50 lbs.


Care Plan and Assessment Functions: Duties


Assist and participate in developing for each resident a preliminary and comprehensive assessment and a written care plan that identifies the nursing and medical problems’ and/or need identified.


Encourage residents and their families to participate in the development and review of care plans.


Ensure that all nursing services personnel are aware of the care plan and that care plans are used in providing daily nursing services to the resident.


Review nurses’ notes and monitor residents to ensure that the care plans are being followed and that residents’ needs are being met.


Safety and Sanitation Requirements: Duties


Monitor nursing personnel to safety standards, fire and disaster, infection control and all other departmental policies and procedures are followed.


Ensure that nursing services personnel follow established infection control procedures when isolation precautions become necessary.


Participate in developing, implementing, and maintaining procedures for reporting hazardous conditions or equipment.


Ensure cleanliness of nursing services work areas, stations, preparation rooms, treatment areas, and resident rooms.
