The incumbent provides comprehensive pre-employment transition services for potentially eligible students with disabilities, ages 14-22.
Provides services to clients in meeting problems of personal, social, and vocational adjustment.
Collects existing information from hospitals/medical records, schools, employers, other agencies, and individuals to supplement and/or verify history and current status.
Evaluates information and its relationship to client's current and potential medical and vocational status.
Determines eligibility for vocational rehabilitation services in accordance with applicable Federal and District laws, regulations, guidelines, policies, and procedures.
Rn To Np Transition Program

Transition Specialists help individuals and families adjust to their new circumstances and develop strategies to cope with the changes. Detailed Job Description A Transition Specialist is responsible for providing support and guidance to individuals and families as they transition from one stage of life to another. They assess the needs of the individual or family and develop a plan to help them adjust to their new circumstances. Transition Specialists provide emotional support, help individuals and families develop coping strategies, and provide resources and referrals to other professionals as needed. They also provide education and information about the transition process and help individuals and families develop a plan for the future. Job Skills Required
