Provide follow up case management to residents that were engaged in the community with the Law Enforcement partner, as needed.
Maintain current knowledge of, and contact with, community agencies to make referrals appropriately
Other job duties as assigned by your supervisor.
One year of postgraduate degree experience in the behavioral health field.
Documents services in a timely fashion meeting all standards and expectations per MCHHS policies.
Provide back up to PCC as needed to include answering phones by logging into Touch Point and conducting mental health assessments.
