Job Description
Department: Transportation FLSA Classification: non-Exempt


Reports To: Facility Director Approved Mgmt – 2021.03 TR-03


Summary/Objective


The van driver is responsible for the safe transport of individuals from their home to facility based services and back home. In addition there may be transportation of clients to meet other needs. Drivers are responsible for the cleanliness and general maintenance oversight of the vehicle assigned.


Essential Functions/Job Duties:


  • Complete all duties and responsibilities in accordance with the agency Code of Ethics.
  • Maintain positive relations and timely communication with agency staff.
  • Other duties related to the mission of the agency and administrative needs as assigned.
  • Represents New Horizons’ mission and values when interacting with families.
  • Provide proper notification in the event of illness or other emergency to allow coverage for the assigned route.
  • Documents passenger health or other concerns on the appropriate form to the Facility Director
  • Complete trip attendance and other records on a timely basis.
  • Maintain compliance to governing authorities including the Bureau of Quality Improvement in Services, Rule IAC 460, Department of Labor, Indiana Department of Transportation, and the agency Quality Management System.
  • Maintain compliance of program requirements, to include training and route schedules.
  • Report mechanical and safety concerns immediately to the facility director.
  • Complete preventative maintenance, safety checks and pre-trip checks as identified and assigned.

Supervisory Responsibility


This position has no direct supervisory responsibilities.


Work Environment


This job operates in the community, in a vehicle. This position routinely uses computers and phones.


Physical Demands


While performing the duties of this job, the employee is regularly required to stand, walk, sit, reach with hands and arms, and talk or hear. The employee is occasionally required to stoop, kneel, or crouch. The employee must lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.


Position Type/Expected Hours of Work


This position may be a part-time to full-time position up to 40 hours / week, based on schedules.


Travel


Travel outside of the job function is not a part of this position.


Required Education And Experience


  • Must have a valid driver’s license.
  • Must have excellent verbal communication skills.
  • Must have an acceptable driving record; 3 points or less for the previous 12 months, or 16 points or less for the previous 36 months (as defined by the Department of Transportation, Motor Vehicle Division (MVD) see Points Assessment.
  • High School diploma or equivalent.
