Job Description
UAS RN Assessor 60% Telehealth and 40% In Home Assessments COME WORK FOR THE LEADING, LOCAL MANAGED CARE COMPANY - VILLAGE CARE! VillageCare is looking for self-motivated, passionate RN's for a Full Time Nurse Assessor. The Nurse Assessor conducts comprehensive assessments of all aspects of potential new members' condition - clinical, social and environmental to determine the level of services needed to appropriately keep member safe in the community. Some daily activities will include: Utilize clinical skills to assess and document all aspects of the potential members long-term community based needs. Provide community-based training and education for frail population care. Conduct face to face UAS-NY assessments according to state guidelines, policies, procedures, and protocols. Gather clinical information from various sources to address all conditions. Communicate with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment. Explain VillageCareMax features and benefits, including explanation of the member's handbook. Consult with supervisor and others in overcoming barriers in meeting goals and objectives. We would like to speak to those who have a current unrestricted NYS RN license along with 1+ year home care, hospice, or field based case management experience preferred. Proficiency in use of UAS-NY required . Prior MLTC/ Managed Medicaid/MAP/FIDA experience preferred. In home visits required 40% of workload. There are many benefits to working for VillageCare.
