Job Description

The Licensed Practical Nurse provides basic therapeutic nursing care according to established standards of practice under the direction and supervision of the RN. Provides care to those patients and caregivers whose care requirements are routine and standardized with predictable outcomes. Assists the RN and healthcare team to achieve desired patient outcomes. Relies on education, training and established guidelines to complete responsibilities. Initially requires instruction and guidance to accomplish job which should develop into working under general supervision.

This is a 13 week contract.

Job Standards - Total Weight 60%

Job Standards and Performance Expectations Weight 1. Resident Care and administering of medications: This will have been satisfactorily performed when:
  • Direct resident care is provided utilizing the nursing process under the direction and supervision of the RN at all times. Diagnostic test results are reviewed and abnormal results and changes in resident condition are reported to the RN, physician and appropriate healthcare team members immediately.
  • Review medication administration records for completeness of information, accuracy in the transcription of the physician's order, and adherence to stop order policies. Dispose of drugs as required, and in accordance with established procedures.
  • Ensure that narcotic records are accurate for your shift. Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift. Prescribed medications, therapies and treatments are administered in accordance with practical nursing standards at all times.
  • Resident medical records, notes and physician orders are accurately reviewed and maintained; All resident care activities are accurately documented in accordance with documentation standards at all times.
  • Components of the plan of care within the LPN scope of practice and competency are implemented to promote desired outcomes at all times.
  • Strictly adheres to infection control and safety policies.
  • The environment to promote resident and healthcare team safety is evaluated and maintained at all times.
  • Compassionate basic nursing care is provided while maintaining resident privacy and confidentiality at all times.

2. Coordination of Care and documentation : This will have been satisfactorily performed when:
  • The execution resident's plan of care is effectively collaborated with the supervising RN at all times. Physicians, RNs and health care team members are effectively assisted with data collection, assessment, planning, intervention and evaluation of resident and families care to achieve desired outcomes.
  • Complete and file required record keeping forms/charts upon the resident's admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physician's Order Form. Chart nurses? notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care. Chart all reports of accidents/incidents involving residents. Follow established procedures.
  • As an advocate for the resident, supports the resident's psychological, ethical, spiritual and cultural needs by collaborating with the RN and healthcare team. Healthcare team members are sought out to serve as a resource to increase practice knowledge
  • Record new/changed diet orders. Forward information to the Dietary Department. Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to Nurse Supervisor. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures. (weekly charting, monthly summaries and unusual events) Sign and date all entries made in the resident's EMR (electronic medical record).
  • Active participation on unit teams, work groups and/ or committees is provided as requested; anticipates needs and voluntarily assists others.
  • Outcomes of the team/committees' work are supported and put into practice; keeps informed of Unit initiatives.
  • Assistance is provided with the orientation of new employees under the supervision of the RN as requested.

3. Quality: This will have been satisfactorily performed when:
  • Knowledge of unit based quality improvement initiatives is demonstrated at all times.
  • Active participation in Quality improvement initiatives is provided by utilizing standards, guidelines, and pathways for care delivery.

4. Nursing Care functions: This will have been satisfactorily performed when:
  • Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care for the dying, etc., as required.
  • Greet newly admitted residents upon admission. Participate in the orientation of new residents/family members to the facility. Make rounds with physicians as necessary.
  • Inform family members of the death of a resident. Call funeral homes when requested by the family.
