Job Description
Overview
Responsible for the review/response/and coordination of care for patients primarily focusing on referrals entered into electronic referral systems. Developing the referral relationships of the agency within the community, in accordance with company policies and procedures. Reverse marketing in coordination with the local BD Leader to reinforce messaging, stay top of mind and present information where it may be of value to our customers.
Responsibilities
Serves as the first line of customer service for external and internal customers (CC relationship).
Professionally responds through electronic referral platforms within the specified time requirements.
Meets assigned key performance indicators (KPIs).
Receives and documents patient referrals to home health services through electronic referral systems, ensuring compliance with applicable coverage criteria.
Coordinates with care centers on ability to staff electronic referrals and communicates with care center Intake Coordinators or designee to enter referrals into Homecare Homebase.
Gathers appropriate information to assist the clinical team in scheduling an evaluation visit.
Communicates pertinent patient information to the care center team.
Makes referrals to other community resources as directed by the Director of Operations/designee.
Communicates and follows up with Business Development regarding pending admissions or as directed by Director of Operations.
Performs other related duties as assigned or requested. Knowledge, Skills and Abilities:
Strong Microsoft Office Skills.
Excellent oral/written communication, presentation, critical thinking, and interpersonal skills.
Proven track record of developing and maintain relationships within a competitive marketplace.
Demonstrated organizational skills within a complex, fast-paced environment.
Ability to handle stressful situations/deadlines.
Qualifications
Required
High School diploma or GED equivalent.
Experience working in an environment where timeliness and quality of response were measured and impacted business results. Preferred
Home health or Hospice experience.
Experience customer facing in sales or operations.
