Social Service Designee, Ft, 2500 Bonus

Redeemer Health Care Center, a 4-star rated, 119-bed senior care campus located in Uptown, is hiring a Full-Time Social Service Designee to join our team. In addition to generous benefits, we are offering a $2,500 Bonus!

In this role, you will advocate for our residents in long-term care and family members to ensure they receive high-quality care and services. Social Service Designee responsibilities:

• Provide additional tasks as required.
• Provide valuable social services to residents and their families in medical, emotional, physical, or psychological concerns.
• Conduct and coordinate Admission, provide appropriate referrals and discharge planning of residents.
• Act as an advocate ensuring basic human and resident rights.
• Act as liaison between families, staff, and various agencies.
• Commitment to compassion, excellence and quality care.
• Provides resources and referrals to staff, residents, and family members.
• Assist the team in coordinating social services for the facility, individualized plans for the residents, medical equipment, counseling or other follow-up treatment plans.

Social Service Designee skills and requirements:

• Successful candidates will be mission-minded with excellent communication skills.
• Must have knowledge of state and federal guidelines related to long-term care and resident rights.
• Transitional or long term care experience preferred with an opportunity for advancement.
