Position Summary/Essential Duties:
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department, hospital and health system.
- Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
- Provides age specific care to patients/families in all age groups.
- Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
- Provides linkage to community resources and UH network services to facilitate optimal care, and serves as liaison to community agencies.
- Develops plans with attention to individual patient and family goals
- Communicates/collaborates with the work team using a multidisciplinary approach.
- Provides guidance/consultation to peers/team regarding complex psychosocial and /or discharge planning issues/barriers.
- Provides leadership (including: process improvement, coordination of staff orientation, team building, etc.)
- Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.
5. Assists with staff development and provides staff/community education.
Experience & Knowledge:
Hospital/Health Agency experience preferred.

- Theoretical and advanced clinical knowledge regarding health care delivery/psychosocial issues.

Education and Required Experience:
Master’s degree from an accredited school of Social Work
Three to Five years Social Work experience in a health care setting.
Required Credentials, License, and / or Certifications:
- Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board
- LISW-S
Preferred Qualifications/Position Details:
Must have an LISW-S and at least 3-5 years of clinical experience. Previous work with the pediatric population is strongly preferred. Emergency medicine experience is also preferred. The schedule is (3) 12 hours shifts, 11:00am-11:30pm/12:00am, Monday-Friday with rotating weekends and holidays.
