Job Description

The Senior Community Health Promotion Specialist leads multiple public health initiatives to affect policy, systems, and environmental change resulting in the improvement of population health outcomes. This role plans, develops, implements, and evaluates programmatic efforts; cultivates authentic engagement of partners and members of the community; and is responsible for resource and budgetary decisions, assisting and leading processes to secure revenue, negotiate contract documents, and fulfill grant requirements.

Duties

  • Oversees program activities for specific grants and initiatives as assigned, including sub grantee activities, ensuring fidelity of process and execution in all activities.
  • Facilitates and sustains community-based coalitions and supports community engagement efforts with stakeholders.
  • Manages provision of technical assistance leading to adoption of organizational or local policy, systems, and environmental changes that support health in prioritized community settings.
  • Leads program planning, implementation, and evaluation; works in collaboration with program evaluators in data collection and other evaluation-related activities.
  • Serves as subject-matter-expert for all assigned program-related and grant-related matters.
  • Builds and fosters partnerships in the community in support of program mission and work.
  • Develops measurable objectives; gathers, analyzes, and interprets data; and engages in performance management and continuous quality improvement activities
  • Develops and drives comprehensive evaluation and quality improvement plans.
  • Collaborates internally and externally across multiple sectors to provide stakeholders with education on evidence-based strategies and best-practices in program, policy, systems, and/or environmental changes for population health.
  • Coordinates with supervisor to respond to the changing public health environment through problem-solving and adapting programs as needed using change management tools and best practices.
  • Negotiates, navigates, and reaches consensus among differing opinions to promote health through changes to systems, policies, and practices.
  • Organizes program data for use in grant reporting, annual reports, and documents as assigned.
  • Develops messaging and communications aimed at increasing the awareness of prevention initiatives and the importance of health in priority settings.
  • May lead grant-writing processes, negotiation of budgets and implementation plans with funders, and processes to identify and secure funding to address community health gaps.
  • Collaborates with internal programs to advance agency priorities related to the program, its mission, and activities.


Emergency Preparedness and Response:

  • Responds, as required, to support public health emergencies, incidents, and events. Employee participates in all exercises and drills on emergency preparedness and response, as required. Completes trainings identified as appropriate for this level employee and initiates corrective actions and responses pursuant to Federal, State, and Local laws, statutes, and regulations.

  • Thorough working knowledge of the social determinates of health, principles and practices of community outreach, disease prevention, program planning; the principles of population-based health promotion and disease prevention; principles and practices of coalition building and developing community partnerships; developing community outreach strategies.

Communicate effectively, both verbally and in writing, to make information, education, and persuasive presentations internally and externally. Ability to use active listening as well as excellent verbal and non-verbal communication skills.

  • Develop and maintain effective working relationships with other County employees, representatives of other agencies and organizations, and members of the community.

Requirements

Education and Experience:

  • Bachelor’s Degree in public health, health policy, community health education, social sciences, epidemiology, nutrition, dietetics, or related field.
  • 8+ years’ experience in health promotion or related public health field.
  • Any equivalent combination of education and work experience that satisfy the requirements of the job will be considered.

Pre-Employment Additional Requirements:

  • Must successfully pass pre-employment testing which includes an acceptable motor vehicle record (MVR) and background check.

WORK ENVIRONMENT:

  • Work environment is a mix of general office environment and engagement with stakeholders and partner organizations in community settings.

PHYSICAL DEMANDS:

  • Spends approximately 75% of time sitting and 25% of time upright, walking, driving, and/or utilizing public transit.
  • Constant use of eye, hand and finger coordination enabling the use of automated office machinery or equipment.
  • Occasionally uses cart, dolly, or other equipment to carry in excess of 20 lbs.
  • Occasionally climbs, stoops, kneels, balances, reaches, crawls and crouches while performing office or work duties.
  • Verbal and auditory capacity enabling constant interpersonal communication through automated devices, such as telephones, radios, and similar; and in public meetings and personal interactions.
  • Visual capacity enabling constant use of computer or other work-related equipment.
  • Occasionally lifts, carries, pulls or pushes up to 20 lbs.
