Job Description

Required Qualifications:

  • Graduate of an accredited school of nursing
  • Current RN licensure in Wisconsin within two months of hire
  • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable
  • Current RN licensure in Minnesota
  • Strongly demonstrated written and oral communication skills

Nursing Process:

Definition:

  • Evaluation includes assessment and reassessment of the patient’s progress toward goal achievement.
  • Nursing assessment is accomplished through interviewing, observation, and/or nursing procedures.
  • Planning is the process of making decisions and setting goals which are mutually acceptable to the nurse and all pertinent others.
  • Problem identification defines the impact of health problems affecting individuals and families. Registered nurses distinguish between normal and abnormal findings in order to identify patient capabilities and limitations.

Care Coordination
:

Definition: Care Coordination is the ongoing support of patients and their families over time as they navigate care and relationships. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.

Performance is up to standard when:

  • Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care
  • Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.
  • Identifies and secures patient services needed for patient care
  • Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans
  • Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention


Education
:

Definition: Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education.

Additional Information:

Anticoagulation and Medication Refill: Minimum 1 year recent experience in Anticoagulation/Thrombosis Care, Cardiology, Hematology/Oncology, Med-Surgical/Acute Care, Internal Medicine or Adult Family Practice. Cardiology: Minimum of 1 year recent experience in Cardiology with experience in CCU, Telemetry or Cath Lab. ACLS certification or obtain within 6 months of hire.

Diabetes program: Must be a certified Diabetes educator or obtain certification within 1 year.

Eye Care: Minimum 1 year recent experience in an Eye Care Clinic with Retina Specialty. Gastroenterology: Minimum 1 year recent GI, Med-Surgical, or SICU experience. ACLS certification or obtain within 6 months of hire. Completion of a Basic ECG Rhythm Class or completion within 3 months of hire.

Infusion: Minimum 1 year recent IV experience. Maternal Fetal Medicine (OB): Minimum of 1 year recent experience in an ambulatory maternal fetal medicine clinic with current clinic knowledge as it relates to Maternal Fetal Medicine.

Nephrology: Minimum 1 year recent experience in Nephrology, Transplant, Dialysis, Cardiology, Medicine Specialty, Internal Medicine or Family Practice.

Neurology: Minimum 1 year recent experience in Neurology, Critical Care, Medical Specialty, Internal Medicine or Family Practice.

Occupational Health: Minimum of 1 year recent experience working in Occupational Medicine. Must obtain certification in Audiometry and Spirometry within 6 months of hire, however, may not be required for some locations.

Orthopaedic Joint program: Minimum one year of recent experience working directly with total joint patients within an orthopaedic practice setting with working knowledge of rehabilitative protocols associated with total joint replacement surgeries.

Plastic and Hand Surgery – Aesthetic: Current Certified Plastic Surgery Nurse (CPSN) and/or Certified Aesthetic Nurse Specialist (CANS) through Plastic Surgical Nursing Certification Board (PSNCB) or pass the certification test within 6 months of hire and maintain the certification.

P M and R: Minimum 1 year recent experience in P M and R, Critical Care, Medical Specialty, Internal Medicine or Family Practice.

Pulmonary: Minimum 1 year recent experience in Pulmonary, Critical Care, Medical Specialty, Internal Medicine or Family Practice.

Travel Medicine: Minimum 1 year recent experience in a Travel Clinic setting, Infectious Diseases, Medical Specialty or Internal Medicine.

Urgent Care: Minimum 1 year recent acute care experience in ambulatory or inpatient settings. Complete competencies in IV administration, catheterization and triage within 6 months of start

Surgery (not including Ortho): Minimum 1 year recent experience in surgery and a working knowledge of infection control and surgical procedures Wound Care: Minimum 1 year recent experience in surgery or a wound clinic and have a working knowledge of ostomy and wound care. Recent experience is defined as within the past 5 years
