Rn Health Coach Jobs

Astera Health currently has a Registered Nurse (RN) Health Coach position open at the Wadena campus. In this role, you will be responsible for taking a transformative approach to care provision that brings together the patient, health care providers and community resources in planning, implementation and evaluation of comprehensive healthcare. The coordinator ensures that patients adhere to cross-continuum care management that results in improved health and the prevention of disease exacerbation.

This position is 48 hours per biweekly pay period, Monday – Friday 8:00am – 4:30pm. Early morning or late afternoon meetings may adjust start and end times of working hours, but would know well in advance. No nights, no weekends, no holidays.

Responsibilities of this position include, but are not limited to:

• Facilitates mutual goal setting and goal directed interactions with the client/family
• Works with patients and families on self-management support
• Provides education and serves as a patient advocate
• Monitors, evaluates, and notates trends in patient responses and identifies risk issues, variances, and improvement opportunities
• Identifies patients not meeting clinical goals and arranges follow-up services by protocol or as appropriate
• Assumes responsibility and accountability for care plan effectiveness and clinical outcomes
• Supports case management for patients on controlled substances and those with opioid use disorder including medication refills, face to face patient visits, and phone calls to patients
• Assesses the client’s emotional and cognitive status and learning readiness within the client’s environment to augment a holistic plan of care

Competencies Required Include

• Proficient computer and telephone skills
• Understanding of health care system and continuum of care, including sites of care, delivery models, and the roles of various providers
• Familiarity of community resources for qualified patients
• Strong interpersonal and communication skills, both verbal and non-verbal

Education And Experience Required

• Or, an equivalent combination of education and experience sufficient to perform the duties as outlined above
• Minimum of 2 years’ experience in case management
• Current licensure as Registered Nurse (RN) in Minnesota
