Description:

The Clinical Quality, Patient Safety, Risk Management Specialist’s primary objective is to work with health system employees and medical staff to improve internal care management systems so care delivery is safer and more reliable. The position is responsible for identifying internal health system quality and safety issues and analyzing the root causes and underlying factors. The QSR Specialist will work with managers and medical staff leadership to develop process changes and improvements, manage litigation claims and develop QSR educational programs.

This position reports to the Manager for patient safety and risk management and our patient safety officer. This position is a full time position that has oversight of 3 hospital's within our system and works collaboratively with the other patient safety and risk specialists in order to coordinate safe care and respond to reportable events. Additionally supports the accreditation leader to ensure compliance with Joint Commission of Health Care Organization standards for hospitals and outpatient areas.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

  • Working knowledge of laws, regulations, accrediting, licensing, monitoring agency standards and requirements applicable to medical staff, risk management and safety.
  • Bachelor’s degree in Nursing and active Michigan Nursing License is required.
  • Four or more years of progressive health care management experience in medical staff issues, clinical practice areas and/or quality/risk management.
  • Certified Professional in Healthcare Risk Management (CHPRM) certification required OR achievement of CHPRM certification within 2 years of becoming eligible.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Develops and provides ongoing reports to senior leadership and the Quality Committee of the Board.
  • Collaborates with quality representatives in developing Performance Improvement Plans, indicators and goals.
  • Develops, implements, and oversees the Serious Reportable Event function and process.
  • Presents and/or reports risk and safety issues, outcomes and recommendations. Reviews all near miss or actual patient adverse event incidents.
  • Member of the appropriate clinical and non-clinical committees within the organization and Trinity Health to provide risk management/patient safety input.
  • Facilitates investigation into significant complaints, working with applicable staff for resolution.
  • Identifies major trending patterns and collaborates on corrective action utilizing the incident/occurrence reporting procedure.
  • Plans, develops and presents facility wide educational programs for Medical Staff, Clinical and Non-clinical associates, residents and students pertaining to patient safety, and risk management.
  • Reviews daily all incident reports submitted and works with applicable managers as needed for review/investigation of reports.
  • Serves as local patient advocate for patients/families.

    • Serve on Ethics Committee, assisting with Ethics consults and education.

  • Tracks and trends key data elements related to patient and visitor safety and risk management.
  • Coordinates the initial investigation of potential or actual adverse events, facilitates Root Cause Analysis of events.
  • Collaborates with the appropriate leadership to establish implementation timelines and monitors the closure of the corrective action plan.
  • Facilitates proactive risk assessment and reduction via tools such as Failure Modes and Effects Analysis.
  • Serve on Ethics Committee, assisting with Ethics consults and education.
  • Oversees proper maintenance of the reporting system, summarizes all occurrence and incident reports and assigns proper follow up.

REQUIRED SKILLS AND ABILITIES

  • Knowledge of basic legal terminology and trial proceedings desirable
  • Proficiency in conducting root cause analyses, defect analyses and debriefings.
  • Demonstrates leadership ability in relating to health professionals and management.
  • Training in statistics, data trending and evaluation preferred.
