Rn - Admit Nurse

As the Admissions Nurse, you will have the opportunity to collaborate with our Director of Clinical Services and clinical teams to contribute to that mission. These passionate team members will depend on you to perform initial terminal diagnosis impact assessments on the patients physical, functional, psychosocial, and environmental needs and daily living.
On an average day, you will

• Manage patient discomfort and provide symptom relief
• Implement individualized plans of care and recommend revisions to plans as necessary.
• Consult with and educate patient and family regarding disease process, self-care techniques, end-of-life care, and the process for dealing with issues of ethical concern
• Assess cultural and spiritual implications, verbal and non-verbal communication patterns, and risk for pathological grief
• Determine scope and frequency of services needed based on acuity and patient/family needs of initial assessment and re-assessments
• And perform additional, related duties as needed
