Job Description


Position Title: Resident Care Assistant - Fieldstone


FLSA STATUS: Non-Exempt


REPORTS TO: Director of Fieldstone and Stonebrook


SUPERVISES: N/A


DEPARTMENT: OMHC

POSITION SUMMARY:


The Fieldstone Resident Care Associate provides the special service and care required to each ALZ / Dementia resident. Provides resident care and program/activity needs to residents while assisting in maintaining a positive physical, nutritional and social environment for the residents, while ensuring they feel safe and purposeful.


Staff value the person first, knows him or her as an individual, and sees the dementia as a disability of certain parts of the brain rather than thinking of the person in terms of their disease and symptoms first.


Ensures shift report is clearly communicated to oncoming shift and communicates issues/ideas openly to fellow lead aids, nurse and Program Director. Acts as a role model for peers in demeanor, appearance and work standards, and remaining professional in all communication. Reacts calmly and efficiently in emergency situations.


  • MUST HAVE CNA or HHA Certifications.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.


  • Demonstrates knowledge of residents in regard to background, participation needs, physical abilities, interests, cognitive capacity and attention span capability.
  • Uses proper sanitary procedures to wipe tables, and sweep dining room and kitchen after meals.
  • Supports the resident’s family, friends and significant others as ongoing caregivers and members of the Caregiving team.
  • Effectively handles difficult resident behaviors.
  • Assures resident information is communicated to the team through the report process and communication log (i.e. appointments, outings, etc.).
  • Participates in service plan updates and introductory visit updates.
  • Provides ongoing supervision of residents health related activities: complies with health care instructions explained by health care professionals and promptly and appropriately seeks and obtains medical attention when needed.
  • Can measure and accurately record vital signs (temperature, pulse, blood pressure ) as directed by nurse.
  • Knows and complies with all resident rights.
  • Initiates programming and activities with residents, leading one-on-one, small group and large group activities.
  • Demonstrates knowledge of the cognitive, physical, behavioral and psychosocial approach to all aspects of dementia by using a sensitive approach to all aspects of resident care.
  • Reports resident and family concerns and issues to supervisor.
  • Checks common areas ¾ including bathrooms ¾ for cleanliness.
  • Checks all residents daily for cleanliness, appropriate clothing, and any continence issues.
  • Successfully follows and implements the plan of care.
  • Supports the resident’s abilities by understanding that all aspects of care can be viewed as an activity
  • Observes and reports any changes in resident’s condition (behavior or health), takes action to ensure safety of the resident and report to supervisor.
  • Serves residents food as directed, following dietary guidelines. Facilitates the Fieldstone Enhanced Dining program, and provides snacks and hydration between meals and as needed.
  • Participates with program staff and residents in daily programs and recreational activities as directed.
  • Encourages residents to participate in daily activity program.
  • Documents residents’ material needs (i.e. briefs, clothing, toothpaste, pads, etc.) and communicates to family.
  • Assists residents with medication per state regulations.
  • Reacts calmly and effectively in emergency situations.
  • Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other).
  • Attends staff meetings on a regular basis.
  • Checks on resident rooms daily for general cleanliness and neatness; ensures high risk rooms are double checked.
  • Provides appropriate interventions to maximize the resident’s social and ADL functioning
  • Responds to emergencies and personal emergency response system and knows when to call for backup.
  • Efficiently documents and provides accurate record-keeping as appropriate.
  • Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transfer, mobility, and continence care while using appropriate modeling and validation techniques
  • Demonstrates therapeutic conversation and redirection conversation with residents, offering appropriate cues for participation in engagement and dining programs.
  • Completes annual in-services (14 Hours/6 Memory Care/50% Live)
  • Accurately completes incident reports prior to the end of shift and reports all incidents to supervisor.
  • Recognizes and reports to Program Director or Nurse a resident’s change of condition, including needing more time to deliver care due to the aging process.
  • Receives and gives report of on-going shifts by oral and written shift report.


Educational Requirements and Experience:


  • Good judgment and decision making.
  • Must possess a current Certified Nursing Assistant or Home Health Aide in accordance with the Massachusetts State regulations - required
  • Reliability including reporting to work as scheduled and on time, and completing assignments according to facility policy.
  • Patience, tact, and a willingness to take direction and to work as a team member
  • Communication skills including the ability to communicate with residents, families and team members.


PHYSICAL REQUIREMENTS/WORKING CONDITIONS:


Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 45 pounds.


  • Possible exposure to chemicals as identified in the MSDS Manual.
  • Possible exposure to unpleasant odors.
  • May be subject to angered and/or emotionally upset family members, personnel and visitors.
  • Possible exposure to residents who may be ill, confused, irritable and irrational.
  • Possible exposure to infections, diseases and blood borne pathogens.
  • May work beyond normal working hours and on weekends and holidays when necessary.
  • Work indoors in a climate controlled setting.
