Under general supervision of the Community Systems Director (reviewed by the Program Nurse Manager), focusing on the social determinants of health (SDOH) and Health Equity, this position will assess, plan, implement, and evaluate needs relative to community/clinical linkages in Lowcountry Region; they will serve as an extension of assessed clinic services & programs as identified by data, provide general outreach through assessments, health screenings & education, provide training and practice consultation to other HD programs as needed, assist with community/clinical partnerships, and coordinate connections between community & clinical sectors to improve population health.


Providing essential input to interdisciplinary programs that monitor, anticipate, and respond to public health problems in population groups. Providing health education, care management, and primary care to individuals and families who are members of vulnerable populations and high-risk groups.

Working with the Community Systems team to assess and establish a more effective and efficient resource referral network for a seamless continuum of care, reduce the duplication of services, and increase access and compliance to services not provided by DHEC by establishing and facilitating a Multi-disciplinary Assessment Team for Regional Integrated Community Services or MATRICS. Applying their expertise and training to provide consultation to the Regional Training Center for community/clinical programs as needed or requested; actively participate in the monthly School Nurse Call and provide consultation to HR/Recruitment Coordinator for enhanced PH Nurse recruitment.

Provides a critical linkage between epidemiological data and clinical understanding of health and illness as it is experienced in peoples’ lives and surveillance and monitoring of disease trends within the community. Utilizing evidence-based strategies, coordinate community health screenings and other community-based activities to promote healthy behavior via collaboration with the Region Mobile Clinic, Community Partner Clinics, and Outreach/CHW events, coordinates findings and services with other disciplines, agencies, and providers through referrals of identified concerns.

Monitoring crucial health status indicators such as environmentally caused illnesses, immunization levels, infant mortality rates, and communicable disease occurrence, to identify problems that threaten the public’s health and develop effective interventions.

Assists with Medical Needs Shelter (MNS) operations, and maintains supportive nursing practice competencies to include, but not limited to, intramuscular, subcutaneous, and intradermal best practice injection administration, oral medication administration, and client emergency response including First Aid, CPR and AED, and be willing and available to assist during times of public health emergency and outbreak response efforts.

Other duties as assigned.


State Minimum Requirements: Licensure as a registered nurse by the State Board of Nursing for South Carolina. Graduation from an accredited school of nursing.


Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.


Agency Additional Requirements: Associate Degree in Nursing and current license as a registered nurse by the State Board of Nursing for South Carolina.

Knowledge and skill of techniques and practices of the nursing profession, in use of the nursing process in health maintenance and health promotion. Knowledge and skill in public health practice and of CQI/QA concepts. Ability to exercise independent judgment while adhering to standing orders, policies, and procedures. Ability to communicate effectively, function as a team member, do health teaching and counseling and interact sensitively, effectively, and professionally with persons from diverse cultural, socioeconomic, educational, racial, ethnic, and professional backgrounds and persons of all ages and lifestyles preferences.


Has a current valid driver's license or ability to obtain transportation. May be required to use a personal vehicle. CPR/First Aid certification within first 6 months and then as required. Able to lift 30 pounds and stand for long periods. May be required to work before 8:30 am, after 5:00 pm or weekends and evenings. May be required to travel to other sites within the region/state.


Drives to community settings throughout the region/state. Carries, loads and sets up audio visual equipment and materials. Educational needs of the target population must be assessed and met in a manner which is sensitive to the abilities, culture, age, gender and other learning characteristics of the customer. Requires visual, auditory, written and oral communication skills.

CURRENT NURSING STUDENTS WILL BE CONSIDERED:
Current nursing students or recent graduates will be considered prior to graduation and/or obtaining their nursing license should they meet all position requirements and provide proof of the obtained degree, transcripts, and nursing license within six (6) months of applying. Otherwise the offer will be rescinded.

Immunization: All new employees who are healthcare providers are required to provide documentation of immunity or be immunized against Measles, Mumps, Rubella, Pertussis, Varicella, and Hepatitis B prior to beginning employment.

State Disaster Plan: In accordance with the State's Disaster Plan, which includes hazardous weather, DHEC employees may be required to work in times of an emergency or disaster.

Driving Record: If this position requires the applicant to possess a valid driver's license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
